La enfermedad de Erdheim Chester es una afección sistémica poco frecuente considerada en la actualidad de naturaleza neoplásica debido a la proliferación monoclonal de histiocitos. Se presenta el caso de un hombre de 31 años con historia de cefalea y tomografía computada de cerebro con lesión ocupante de espacio en región fronto-parieto-temporal derecha por lo que se realizó biopsia excisional. Los hallazgos fueron compatibles con proceso histiocitario en relación con desorden xantogranulomatoso/histiocitosis no Langerhans/enfermedad de Erdheim Chester (ECD). Se solicitó PET-TC evidenciando en topografía de retroperitoneo izquierdo una voluminosa lesión hipermetabólica con densidad de partes blandas, heterogénea y bordes lobulados. El PET CT con F18 FDG tiene un papel fundamental al diagnóstico, ya que ayuda a determinar la extensión de la enfermedad, identificar posibles sitios de biopsia, y también es de utilidad para evaluar la respuesta al tratamiento.

Hypertrophic pachymeningitis associated with antineutrophil cytoplasmic antibody–associated vasculitis is a rare dural inflammatory disorder. We describe MRI and FDG PET/CT findings in 3 cases of hypertrophic pachymeningitis associated with antineutrophil cytoplasmic antibody–associated vasculitis. Enhanced brain MRI of the 3 cases showed linear enhancement of the thickened dura matter involving the skull base, tentorium, and/or convexity. On FDG PET/CT, the thickened dura matter showed diffusely increased FDG uptake with SUVmax ranging from 5.8 to 11.3. Familiarity with these MRI and FDG PET/CT findings is helpful for correct diagnosis and treatment.
