Aetiopathogenesis of severe cutaneous adverse reactions (SCARs) in children: A 9‐year experience in a tertiary care paediatric hospital setting

Abstract: Background: Severe cutaneous adverse reactions (SCARs) are delayed-type hypersensitivity reactions to drugs including as follows: Drug Reaction with Eosinophilia and Systemic Symptoms (DRESS), Stevens-Johnson syndrome (SJS), Toxic Epidermal Necrolysis (TEN) and Acute Generalized Exanthematous Pustulosis (AGEP). Incidence, triggers and management of SCARs have not been investigated in large-scale epidemiological studies on children. Objective: The aim of our study was to collect epidemiological, clinical and aet… Show more

“…The LTT can help identify causes of SCARs, when DPT is strongly contraindicated. SCARs in 54 children were recently investigated, and LTT sensitivity towards BL or carbamazepine was reported up to 80%, 59 although a study on adults revealed lower sensitivity (25%‐75%), and the performance of LTT is influenced by the culprit drug and the type of reaction 60 . Specifically, positivity is particularly high when the LTT is combined with the measurement of cytokines and cytotoxic markers (cyto‐LTT) 61 .…”

“…Another distinguishing feature between the two types of exanthema is dermal eosinophilic infiltration, which was also found in a significantly higher number of biopsies from DIE (36%‐62.5%) 35,57‐60 compared with VIE (16.6%‐20%) 36,66 . It was noted, however, that 12.5% of DIE and 20.8% of VIE biopsies showed no infiltrate 36 …”

“…The aim of the present study is to analyse the clinical features of a cohort of children with MP‐associated mucocutaneous disease, by performing an allergy workup in order to define whether drugs could have an additional role in the pathogenesis of such diseases plus providing the clinicians with the tools for a correct and complete diagnosis. In fact, in these cases, a concomitant infection can be seldom found, so that cutaneous manifestations are the result of a complex interaction of factors 10 …”
