Aetiology of Community-acquired Pneumonia in Hospital Treated Patients

Abstract: From May 1982 a prospective 1-year study of adult patients with community-acquired, radiologically verified, hospital treated pneumonia was performed at the Department of Infectious Diseases, Orebro Medical Center Hospital, Orebro, Sweden. The study included 147 patients with a median age of 71 years. Special efforts to diagnose a pneumococcal aetiology were accomplished by antigen detection of the pneumococcal C-polysaccharide (PnC) in sputum and saliva samples and by serological methods for determination of … Show more

“…In Denmark, until very recently, pneumococcal vaccine was used only in asplenic patients. All studies of aetiology show that the majority of community acquired pneumonias are caused by pneumococci [2,6,7]. Recent studies suggest that the vaccines now available are effective in preventing this type of infection in elderly subjects with chronic diseases [26,27].…”

“…Although there are numerous papers dealing with causes of community acquired pneumonia [2,[5][6][7] and with the prognosis once the patients have been admitted to hospital, the literature concerning the identification of factors that make subjects prone to develop pneumonia is sparse. In fact, we are familiar with only one prospective study, where subjects from the general population have been followed with regard to pneumonia mortality and morbidity [8] and a few other studies that investigated cohorts of employees or ambulatory patients [9,10].…”

“…The agents causing community-acquired pneumonia in patients treated in hospital have been shown to be Streptococcus pneumoniae, Haemophilus influenzae, Mycoplasma pneumoniae, Chlamydia species, Legionella species, and viruses with geographical variations [1,2]. Since a definite etiological diagnosis is obtained in approximately 50% of patients with pneumonia in clinical studies and in fewer in the hospital routine, other and preferably rapid differential diagnostic tools are needed.…”
