Aetiological factors related to a positive direct antiglobulin test result in human immunodeficiency virus‐infected patients

Abstract: Significantly lower Hb levels were detected in DAT-positive HIV(+) patients. HCV co-infection and TMP-SMX prophylaxis appear to confer an increased risk of DAT positivity. The presence of red blood cell autoantibodies may be associated with anaemia in HIV disease in the absence of overt haemolysis.

“…By comparison, DAT positivity was significantly associated with TB incidence in both univariate and multivariate analyses (OR ϭ 11.96 [4.68 to 30.93] and 12.65 [3.33 to 52.75], respectively; both P Ͻ 0.001). Therefore, extending previous findings on the association between anemia and DAT (16,22), our data provide evidence that increased anti-RBC autoantibody production in HIV is associated with increased TB risk.…”

“…Anti-RBC autoantibodies, which sensitize RBCs for destruction, have also been implicated in HIV-associated autoimmune hemolytic anemia (39,40). A positive DAT result and anemia are frequently observed in the clinical syndrome of HIV infection, and these two parameters are significantly associated with each other (16,22). Additionally, the severity of anemia can be used to predict the incidence of and mortality associated with TB in patients with HIV (21).…”

“…16 In a study by Lai et al to evaluate DAT in human immunodeficiency virus (HIV)-infected individuals, hepatitis C virus coinfection was shown to confer an increased risk for DAT positivity. 17 The patients with hepatitis C infection in the present study were not on any treatment. High immunoglobulin levels have been shown to be associated with positive DAT.…”

“… 14 15 Chronic lymphocytic leukemia and hepatitis C infection are known to be associated with cold agglutinin syndrome causing hemolysis. 16 17 Development of autoantibody as well as alloantibody is known to occur in transfusion-dependent thalassemia major. 35 In the patients who had received blood transfusion in the past 3 months, alloantibody directed against the transfused red cells is likely to be associated with positive DAT and positive autocontrol.…”
