Aesthetic Correction of Tuberous Breast Deformity-Lessons Learned with a Single-Stage Procedure

Abstract: Tuberous breast deformity is a pathologic condition of the breast affecting teenage women. The aberration of breast shape in tuberous breast deformity consisting of a constricting ring at the breast base, breast tissue deficiency, and herniation of breast tissue into the nipple-areola-complex with areola enlargement and additional asymmetry makes the surgical correction challenging. In the present study, 15 patients were operated using a surgical procedure, which addresses all aspects of the deformity at a one… Show more

“…Asymmetry is common finding in tuberous breast deformity and it is very much related to discrepancies as regards; breast volume, shape, areola size, and degree of ptosis. We agree with Bach et al, [32] and Chen et al, [33] that breast symmetry especially breast shape is important and hard task to achieve. “…This type of scoring was done for two reasons; to effectively release the constricting ring and also to expand the sparse glandular tissues to cover the breast implant. Scoring was done by many authors with different techniques; performing radial incisions of mammary base [1], full-thickness incision through skin, subcutaneous tissue, and breast [6], horizontal transection of the constricting ring and using the inferior portion for auto-augmentation [8], vertical transection of constricting at 6 O'clock and created two pillars sutured in a "double-breasted" fashion in the inferior part of the breast [3,32] or dividing the gland to the pectoralis fascia and performing radial scorings [15].…”

“…4,5,7,8 Other authors supported the hypothesis of strong adherence between the dermis and the deeper muscle layer and presence of the constricting fibrous ring at the glandular base, preventing radial expansion and promoting a more protuberant and tubular shape of the glands. 9,10 Another theory suggests an implication of hormonal action, which normally promotes breast enlargement during puberty. In particular, estrogenic hormones normally induce horizontal growth of the galactophorous ducts with a horizontal widening of the NAC.…”

“…From the beginning on, SBI associated with certain clinical local side effects, such as pain, capsular contracture, implant rupture and silicone leakage. Many women still undergo a breast augmentation with SBI in aesthetic and reconstructive surgeries, i.e., after mastectomy [6][7][8]. SBI are supposed to be associated with atypical systemic symptoms such as myalgia, arthralgia, fatigue, fever, dry eyes and/or dry mouth (sicca), as well as cognitive disturbances [9,10], termed as the condition ''Autoimmune/inflammatory syndrome induced by adjuvants'' (ASIA) [11].…”
