Aesthetic assessment in periciliary “v-incision” versus conventional external dacryocystorhinostomy in Asians

Abstract: "V-incision" external DCR has a similar functional success rate to that of the conventional approach and has superior aesthetic outcomes as reported by surgeons and patients. However, a higher proportion of trainees under supervision performed conventional DCR, and it is uncertain whether the outcomes were also influenced by the level of surgeon's expertise.

“…However, END-DCR has some advantages over EXT-DCR such as limited invasiveness, shorter operative time, preservation of pump function, less bleeding, and absence of skin trauma that can lead to an external scar. 1 Recently, Ng et al 32 described a new surgical EXT-DCR technique in which the external cutaneous scar can be avoided, thanks to a periciliary incision, making the procedure more aesthetically desirable, with a functional success rate of 83.3%. Similar aesthetic external surgical approaches have been proposed by other authors with variable results.…”

“…The total protein concentration (TPC) of tear fluids is 7-fold lower than that of blood serum. 15 Therefore, highly sensitive molecular techniques including immunoassays, 16 electrophoresis, 17 and mass spectrometry 18 can be useful in tear fluid analysis. However, only small volumes of tear fluids can be obtained from normal, nonirritated eyes.…”

“…20 Some surgeons elect to apply topical mitomycin-C to the intranasal rhinostomy site. 8 Mitomycin-C is an antimetabolite that inhibits fibroblast function and has been used to modulate postsurgical fibrosis in a variety of applications. 17 However reports on the utility of mitomycin-C in prevention of postoperative mucosal fibrosis and rhinostomy closure have demonstrated mixed results.…”

“…steeper learning curve. 8 Endonasal techniques were also limited because they provided poor visualization of the lacrimal sac in the superior nasal cavity and lacked effective instrumentation to adequately open the sac. 15 This may result from fibrous occlusion of the rhinostomy site or the presence of synechiae between the lateral nasal wall and middle turbinate or nasal septum.…”
