Background Advances in surgical techniques have significantly improved the safety of thyroidectomy. Quality of life issues, such as voice changes, however continue to be important considerations. This study evaluated the prevalence and determinants of voice changes following thyroidectomies for non‐malignant goiters in a Nigerian Hospital. Methodology Consecutive adults who had thyroidectomy for non‐malignant goiters were evaluated. Preoperatively, each participant had laryngoscopy and voice assessment using the Voice Handicap Index‐10. At surgery, the intubation difficulty score, size of endotracheal tubes, and duration of operation were recorded. Laryngoscopy and VHI‐10 assessments were repeated 1 week, 1 month, and 3 months after surgery. Pre and postoperative assessments were compared. Result Fifty‐four patients completed the study. Subtotal thyroidectomy was the commonest operation performed in 17 patients (31.48%), with 14 (25.93%) having total thyroidectomy. Baseline median VHI‐10 score was 6.5(IQR‐10). Five patients had abnormal preoperative laryngoscopy findings (benign vocal cord lesions). Postoperatively, seven patients had endoscopic evidence of vocal cord paralysis, six were unilateral while one was bilateral. Ten patients (18.5%) had early voice changes. Median VHI‐10 score at one week was significantly higher than baseline (10 vs. 6.5, p = 0.01). At 3 months, VHI‐10 reverted back to baseline in five of the ten patients while five patients (9.3%) had persistent voice changes. Abnormal endoscopic findings predicted the occurrence of early postoperative voice changes. Conclusion Thyroidectomy for non‐malignant goiters is associated with transient voice changes, many of which revert to normal over time. Abnormal endoscopic findings predict its occurrence.

The Effect of Visual Feedback on the Voice Control Ability of Pre-lingual Deafened Young Adults with Cochlear Implants

The purpose of this study is to investigate whether there is a difference in the voice control ability between young adults with cochlear implants (CIs) and a normal hearing (NH) group. This study examined the correlation between CI-related characteristics, performance, and acoustic measurements (i.e., voice dynamic area, voice range profile slope). Methods: Participants were 12 young adults (8 females and 4 males) who had CIs before 7 years of age, and 12 NH young adults who matched their sex and age. A voice range profile (VRP) program was used to evaluate voice control ability according to the presence or absence of visual feedback. Results: The voice dynamic area (VDA) of the NH group was significantly larger than that of the CI group, and the VDA was significantly greater when the visual-auditory feedback was presented rather than relying solely on hearing. There was no significant difference between the groups in the VRP slope, but the slope was significantly smaller when the visual-auditory feedback was provided than when only the auditory feedback was provided. There was a negative correlation between the consonant accuracy and the VRP slope in the CI group. Conclusion: Congenitally deafened young adults didn't achieve normal voice control abilities in comparison with NH. However, CIs were able to use the visual information in order to better control their voice. Additionally, the VDA is appropriate as a variable to measure the voice control ability of CIs, but the VDA and the VRP slope are complementary and need to be considered together when analyzing the results.

Breathing is the voice's driving force and also acts as a regulator of larynx function and efficiency. Respiratory distress is a side effect of general anesthesia in thyroid surgery. Therefore, this study's objective was to provide practical and complementary information for voice recovery after thyroid surgery, based on aerodynamic evaluation pre-and post-thyroidectomy. From May 2014 to July 2015, aerodynamic evaluations were performed on 34 female patients diagnosed with thyroid papillary cancer one week before surgery (PRE), one month after surgery (P1), and three months after surgery (P3). The Phonatory Aerodynamic System (model 6600, KayPENTAX, USA) was employed for this purpose, and a total of 29 analysis parameters were selected. The results showed statistically significant differences in peak expiratory airflow (p=0.004), mean pitch (p<0.01), expiration airflow duration (p=0.001), and expiratory volume (p=0.018), based on time factors. In the comparison of time factors, peak expiratory airflow and mean pitch parameters were different in PRE-P1 and PRE-P3. Expiration airflow duration and expiratory volume parameters were different in PRE-P3 and P1-P3. The interaction effect of time and surgical range was significant only for expiratory volume (p=0.024). Female patients who undergo thyroidectomy require post-operative breathing training, and exhalation improvement is considered to reflect a positive lifestyle after surgery.
