Advances in the Role of Endothelial Cells in Cerebral Small Vessel Disease

Abstract: Cerebral small vessel disease (CSVD) poses a serious socio-economic burden due to its high prevalence and severe impact on the quality of life of elderly patients. Pathological changes in CSVD mainly influence small cerebral arteries, microarteries, capillaries, and small veins, which are usually caused by multiple vascular risk factors. CSVD is often identified on brain magnetic resonance imaging (MRI) by recent small subcortical infarcts, white matter hyperintensities, lacune, cerebral microbleeds (CMBs), en… Show more

“…The main component of the BBB is represented by BMECs, a special and unique type of endothelial cells found only at the level of the brain microvessels [ 27 ]. Compared to endothelial tissue located in other regions of the human body, the brain endothelium is characterized by a higher selectivity, one of the most important features of the BBB in brain protection [ 28 ].…”

“…Moreover, endothelial dysfunction is also related to the impairment of the BBB and a decrease in CBF, and both of these processes are involved in the development of CSVD. An increased permeability of the BBB causes local microhemorrhages and decreases distal blood flow, which leads to an aggravation of the regional ischemia in the brain [ 23 ].…”

“…Endothelial dysfunction is an important component in multiple disorders with neurovascular component, including Alzheimer’s disease (AD), cerebral small vessel disease (CSVD), ischemic stroke (IS) and migraine [ 22 , 23 , 24 , 25 ]. Thus, the subject of involvement of EPCs, or their dysfunction, in these diseases has been gaining increasing scientific attention in the recent years.…”

“…It is the most common cause of vascular dementia, accounting for 25% of ischemic strokes and most hemorrhagic strokes (Agirman et al 2021 ). It is commonly associated with Alzheimer’s disease and exacerbates the resulting cognitive impairments, which are responsible for approximately 50% of dementias worldwide (Arba et al 2019 ; Bai et al 2022 ). In China, lacunar infarction caused by cerebral microvascular disease accounts for 25 to 50% of ischemic stroke, which is higher than that in Western countries (Bordes et al 2022 ).…”
