Pendahuluan: Metastasis leptomeningeal (ML) merupakan suatu keadaan terjadinya penyebaran sel-sel kanker ke leptomening dan rongga subaraknoid. Keadaan ini merupakan kondisi emergensi dengan insiden yang semakin lama semakin meningkat. Prognosisnya juga sangat buruk dengan kesintasan yang rendah, hanya berkisar 10-15 minggu. Tujuan: Untuk menentukan kesintasan pada pasien ML yang dirawat di bagian Neurologi Rumah Sakit Kanker Dharmais (RSKD) dan RSUPN Dr. Cipto Mangunkusumo (RSCM). Metode: Penelitian secara kohort retrospektif berdasarkan rekam medis pasien ML di RSKD dan RSCM pada bulan Januari 2018 sampai Juni 2019. Dilakukan pencatatan hasil pemeriksaan klinis, pemeriksaan CSS, serta hasil MRI dan histopatologi tumor primer. Subjek atau keluarga dihubungi melalui telepon untuk mengetahui masih hidup atau sudah meninggal dunia, yang tidak dapat dihubungi akan dieksklusi. Masing-masing subjek dinilai kesintasannya berdasarkan lamanya waktu subjek bertahan hidup sejak diagnosis ML sampai akhir masa observasi, yaitu sampai berakhirnya masa penelitian. Hasil: Didapatkan 62 orang subjek yang mayoritas perempuan (66,1%). Sakit kepala merupakan gejala yang paling sering ditemukan (46,8%) diikuti oleh gangguan saraf kranialis (45,2%). Median kesintasan didapatkan 13 minggu, tidak ada perbedaan kesintasan antara keganasan hematologik dengan tumor solid. Diskusi: Didapatkan median kesintasan 13 minggu dan tidak ditemukan perbedaan kesintasan yang bermakna antara keganasan yang berasal dari hematologik dan tumor solid. Median kesintasan pada penderita ML dalam penelitian ini tetap buruk dari manapun asal proses metastasisnya. Kata kunci: Keganasan hematologik, metastasis leptomeningeal, tumor solid, kesintasan

Diffuse leptomeningeal glioneuronal tumor has various clinical manifestations and is easily misdiagnosed. This article aims to describe the clinical features of a patient with diffuse leptomeningeal glioneuronal tumor misdiagnosed as optic neuromyelitis lineage disease (NMOSD), analyze the causes of misdiagnosis, and identify the disease with similar clinical manifestations. Adolescent women with decreased vision and headache, nausea, and vomiting were initially diagnosed as NMOSD. Immunotherapy was ineffective; enhanced MRI and cerebrospinal fluid cytology examination were completed and compared with classic meningeal carcinoma, NMOSD, multiple sclerosis, and Vogt-Koyanagi-Harada (VKH) syndrome. Diffuse leptomeningeal glioneuronal tumor was finally diagnosed in adolescent women with no history of tumor. Compared with NMOSD: no history of infection, no AQP4, MOG antibody, no spinal segmental abnormalities, immunotherapy was ineffective; compared with multiple sclerosis: no history of infection, no plaque, no oligoclonal antibody, immunotherapy was ineffective; compared with VKH syndrome: no history of infection, no fever, no treatment for immunotherapy; and compared with classic meningeal carcinoma: adolescents, no extracranial tumor lesions, super high protein, more different sugar excess. Patients with acute visual impairment should consider not only NMOSD, but also diffuse leptomeningeal glioneuronal tumor (DLGNT) and other diseases. DLGNT could not be excluded when CSF cytology and MRI plain scan were negative. The positive rate of malignant cells can only be increased by repeatedly sending fresh cerebrospinal fluid for examination; DLGNT needs to be differentiated from NMOSD, multiple sclerosis, VKH syndrome, and other diseases. Early diagnosis and molecular targeted therapy can improve the quality of life and prolong life.

Clinical application of cerebrospinal fluid cytology in China

Cerebrospinal fluid (CSF) cytology is a fundamental test for the diagnosis of central nervous system (CNS) disease in neurological practice in China. CSF cytology provides diagnostic evidence in cases suspected of CNS infectious diseases, autoimmune encephalitis and CNS neoplastic disorders. Immunocytochemistry and flow cytometry make the cytological diagnosis more sensitive and specific. However, the diagnostic significance of CSF cytology should be interpreted and evaluated in the context of specific clinical backgrounds. Although molecular diagnostic techniques, including polymerase chain reaction and next-generation sequencing, are increasingly applied to CSF examination, the classical CSF cytology still has its horizon in the era of precision neurology and should be recommended as a routine test for CSF analysis.
