Advanced Strategies for the Regeneration of Lumbar Disc Annulus Fibrosus

Abstract: Damage to the annulus fibrosus (AF), the outer region of the intervertebral disc (IVD), results in an undesirable condition that may accelerate IVD degeneration causing low back pain. Despite intense research interest, attempts to regenerate the IVD have failed so far and no effective strategy has translated into a successful clinical outcome. Of particular significance, the failure of strategies to repair the AF has been a major drawback in the regeneration of IVD and nucleus replacement. It is unlikely to se… Show more

“…Bailey et al used an FDA-approved X-close device to suture annulus fibrosus (AF) defect after discectomy [4]. In addition, commercial annular closure devices such as Barricaid implant and Inclose Mesh were used to block off the IVD defect by forming a mechanical barrier [5,6]. However, these repair techniques focus on closing the wound of the defect without functional restoration of the IVD, and some scholars found that the additional annulus fibrosus suture (AFS) had no significant superiority [4,7].…”

“…Spinal fusion is regarded as gold standard for treating low back pain caused by IVD degeneration. However, it comes with limited spinal mobility and increased chance of adjacent IVD degeneration [ 119 ].Therefore, many efforts and strategies have been placed on the development of engineered IVD to replace the degenerated IVD [ 120 , 121 , 122 ]. In particular, hydrogels including alginate, chitosan, hyaluronic acid, and collagen have been found to have potential as a scaffold in the IVD degeneration [ 121 , 123 ].…”

“…Various complications, such as physiological anatomy change and re-revision surgery, would increase the therapeutic costs and then caused a great economic burden on society. 2 The etiology of IDD was complex and previous studies revealed that increased age, 3 unhealthy behaviors and inflammatory reactions were associated with IDD incidence. 4 Considering the important therapeutic value for the early management of IDD, detecting a potential molecular biomarker would provide both advanced understanding and more potential treatments for the IDD cases.…”
