Adults with Meningitis Caused by Viridans Streptococci

Abstract: Adult bacterial meningitis caused by viridans streptococci is not uncommon and otopharyngeal infection appears to be an important portal of entry. Clinical manifestations varied according to the different underlying conditions. The results of this study also demonstrate the high incidence of intracranial focal suppuration and cerebral vasculitis concomitant with meningitis. Penicillin remains the treatment of choice for patients with meningitis caused by viridans streptococci. Therapeutic outcome is favorable … Show more

“…It is probable that the viridans streptococci had initially caused the above-mentioned focal infections like sinusitis, which are well known predisposing factors that increase the risk of bacterial meningitis. 14 In the present case there was established maxillary sinusitis, which can be the source of meningitis. However, in a third of these meningitis cases the exact source of infection is not clear.…”

“…Furthermore, poor oral hygiene might be a risk factor for S. viridans infections. Our patient had strabismus correction eye surgery; to date, there has been no history of meningitis due to S. viridans after an eye operation; however, endophthalmitis and meningitis due to S. pneumonia have been reported 4,10 …”

“…However, the initial response to flomoxef was not satisfactory, which may be related to poor penetration of flomoxef into the central nervous system. Patients infected by viridans streptococci have usually had a high incidence of focal suppuration and cerebral vasculitis, but had a consistently favourable prognosis (Lu et al , 2001a, 2002; Yaniv & Potasman, 2000). Only the initial consciousness level and the presence of seizures have been reported to be strongly associated with mortality (Lu et al , 2001b).…”
