Adult intensive-care patients with 2009 pandemic influenza A(H1N1) infection

Abstract: In France, the surveillance of hospitalized cases of pandemic influenza was implemented in July 2009 and restricted to intensive-care unit (ICU) patients in November. We described the characteristics of the 1065 adult patients admitted to ICUs and analysed risk factors for severe outcome (mechanical ventilation or death). Eighty-seven percent of cases were aged 15-64 years. The case-fatality ratio was 20%. The risk for severe outcome increased with age and obesity while this association was negative for chroni… Show more

“…A majority of hospitalized patients requiring therapy in ICUs were adults <65 years who on admission had respiratory failure due mainly to primary influenza pneumonia in agreement with other observations,8, 15, 21, 22, 23, 24 and in clear contrast to seasonal influenza. The number of patients that are admitted to an ICU may vary related to criteria used by the institution.…”

“…The number of patients that are admitted to an ICU may vary related to criteria used by the institution. Consequently, the mortality in ICU differs has been reported from 11% to 25% 15, 21, 22, 23, 24, 25. Many patients with severe disease have co‐morbidities, but it is well described that fatalities from pandemic influenza may occur in previously healthy subjects.…”

“…During the last pandemic, experience in the management of the most severely affected patients with influenza A (H1N1)-related acute respiratory distress syndrome (ARDS) included increased use of extracorporeal membrane oxygenation (ECMO) [1,2]. Growing interest in this technique has been spurred by the results of a large cohort study in which transfer to an ECMO center was associated with lower hospital mortality than matched non-ECMO-referred patients [3].…”
