Adult Distal Humeral Metaphyseal Fractures: Epidemiology and Results of Treatment

Abstract: The epidemiology of a consecutive unselected series of adult distal humeral fractures is defined in this study. The majority of these fractures are best treated surgically by rigid open reduction and internal fixation, except for "low" Type A and C fractures, which have a higher risk of union complications. The role of total elbow arthroplasty to treat these more complex injuries requires further evaluation.

“…The treatment of distal humerus fractures is labourintensive and complex, with an expected high incidence of complications [18]. Over one fourth (28 %) of our patients developed a complication after operative treatment, and one required further surgery (hardware removal).…”

“…Although there is still some controversy regarding the treatment of extra-articular distal humerus fractures, many surgeons recommend surgery with plates and screws, [1][2][3][4][5][6][7] while others give precedence to functional bracing, even in the case of a radial nerve lesion. 8,9 Robinson et al 1 reported a better healing response in operated patients with an extra-articular metaphyseal fracture of the distal humerus compared to patients treated non-operatively.…”

“…8,9 Robinson et al 1 reported a better healing response in operated patients with an extra-articular metaphyseal fracture of the distal humerus compared to patients treated non-operatively. Jawa et al 10 found a more significant loss of elbow and shoulder function in non-operated vs. operated patients, and concluded that operative treatment provides a quicker and more complete recovery, warning, however, that it also increases the risk of complications in the form of iatrogenic injuries of the radial nerve and wound infection.…”
