The incidence of cor triatriatum among neonates with congenital heart disease is only 0.1% to 0.4%. The diagnosis in adults is extremely rare. A 68-year-old patient was referred to our cardiothoracic surgery department following two-dimensional transthoracic echocardiography and a diagnosis of cor triatriatum. Echocardiography was performed due to stroke and atrial fibrillation. Echocardiography revealed a membrane in the left atrium, with the typical presentation of cor triatriatum type 1A. We operated the patient and excised the membrane. The operation was completed conventionally with sinus rhythm. Cor triatriatum in geriatric patients is a rare congenital cardiac abnormality and can be a reason for atrial fibrillation and stroke. The treatment of choice is surgery.Key Words: Geriatric; atrial fibrillation; stroke; cor triatriatum Cor Triatriatumlu Geriatrik Bir Hastanın Cerrahi Tedavisi: Atriyal Fibrilasyon ve İnmenin Nadir Bir Sebebi ÖZETKonjenital kalp hastalığı olan yenidoğanlarda cor triatriatum görülme sıklığı %0.1-0.4 olup, erişkinlerde ise görülme olasılığı çok nadirdir. Altmış sekiz yaşında bir hasta ekokardiyografi ile cor triatriatum tanısı konmuş olarak kliniğimize başvurdu. Ekokardiyografik değerlendirme mevcut atriyal fibrilasyon ve meydana gelmiş inme etyolojisini araştırmak için yapılmış olup, değerlendirme sonucunda sol atriyum içinde membran olduğu saptanmış. Değerlendirme sonucunda hastaya tip 1 A cor triatriatum tanısı konmuş. Hasta tarafımızdan opere edilerek mevcut membran çıkartıldı. Operasyon sonrası hastanın ritmi sinüs seyretti. Cor triatriatum geriatriklerde çok nadir görülmekte olup, atriyal fibrilasyon ve inmeye neden olabilmektedir. Bu tip hastalarda tedavi seçeneği cerrahidir.

A 29 year old female patient presented to the cardiology OPD with history of progressive breathlessness of NYHA class II and palpitation of 1 year duration. Under evaluation, she underwent 2D transthoracic echocardiography that revealed an extra septum that subdivided the left atrium into proximal and distal chambers. The diagnosis of cor triatriatum was hence made and was referred to surgical team for corrective surgery. The communication between proximal and distal chamber was provided by large fenestration in the fibromuscular membrane.Nepalese Heart Journal 2017; 14(1): 33-34

A double interatrial septum (DIS) is a rare finding during echocardiographic evaluation and can often be mistaken for other more common atrial anomalies. The interatrial cavity created by the septum creates a low-flow state that increases risk of thrombus formation and thromboembolic events. Transesophageal echocardiography (TEE) plays a vital role in accurate diagnosis as a DIS is often not seen during transthoracic echocardiography (TTE). In this case, we report a patient who presented for surgery with a preoperative diagnosis of an atrial myxoma, was instead discovered to have a DIS, and then subsequently underwent DIS resection without complication. We discuss the differential of a DIS, including an atrial septal pouch, cor triatriatum, atrial myxoma, and aneurysmal interatrial septum and the classic features of each anomaly for appropriate diagnosis and management.
