Adrenal insufficiency secondary to carcinoma metastatic to the adrenal gland

Abstract: Four patients are reported in whom adrenocortical insufficiency developed consequent to carcinoma metastatic to the adrenal glands. Recognition and treatment of this entity may lead to palliation of symptoms and prolongation of useful life. The computerized axial tomographic scanner is a valuable diagnostic aid in diagnosing adrenal enlargement.

“…First, there is a broad spectrum of malignant tumours and metastases which can infiltrate the adrenal glands and the oesophagus. Although infiltration of the adrenal glands is not uncommon for some tumours, about 90% of adrenal tissue has to be destroyed to result in clinical overt insufficiency, which is rarely the case (Sheeler et al, 1983). In our patient however, no malignant tumour was found and the uneventful follow-up also argues strongly against a neoplastic disease.…”

“…It plays a significant role in distinguishing between adenoma, carcinoma, and metastasis in cases of "adrenal incidentaloma". On the other hand, it allows for identification of the functional nature of the adrenal mass when an adrenal incidentaloma is found [18,19] . The emergence of PET-CT for detection and surveillance has allowed for more accurate specifics of adrenal metastases, with better characterizations of the metabolic activities.…”

“…Needless to say, it is essential to examine whether the adrenal mass is functional or nonfunctional when an adrenal incidentaloma is found. 23,24 Moreover, Candel et al 18 also emphasized that fine-needle aspiration biopsy of adrenal masses was helpful for making an accurate diagnosis. Although this procedure was not performed preoperatively in our patient, it is considered effective for the differential diagnosis of adrenal masses.…”
