Adolescent varicocoele management controversies

Abstract: SUMMARYVaricocoele is defined as excessive dilatation of the pampiniform venous plexus of the spermatic cord. Varicocoele frequently appears during early puberty and is recognized to be the most common surgically correctable cause of male infertility. However, the actual incidence in adolescents, pathophysiology and the association with male factor infertility all remain somewhat controversial. The most accurate diagnostic technique for identifying young men who will benefit from surgical treatment has yet to … Show more

“…Most cases of extra-testicular (obstructive) male infertility require surgical interventions. Varicoceles management involves surgical treatment, including laparoscopic procedures, embolisation and laser obliteration [48,49]. Available data demonstrate that the treatment of varicoceles results in satisfactory outcomes: improved quality of sperm is observed in 70-80% of men; however, there is a lack of improvement in 15-20% while deterioration occurs in about 5%.…”

“…Male infertility is a major public health problem that has been increasing in recent decades, at least in Western countries (Carlsen et al ., ; Swan et al ., ). Varicocoele, an abnormal dilation of the veins of the pampiniform plexus of the spermatic cord, is the most common surgically correctable cause of male infertility (Dubin & Amelar, ; Sigman & Jarow, ; Serefoglu et al ., ). It is a common entity among adolescents and young adults with prevalence rates ranging from 3 to 25% in the general population, varying by method of examination, population characteristics and age at examination (Oster, ; Steeno et al ., ; Stavropoulos et al ., ; Kumanov et al ., ).…”

“…Varicocele is clinically defined as excessive dilation of the veins of the pampiniform venous plexus and the testicular veins (Serefoglu, Saitz, La Nasa, & Hellstrom, ). The prevalence of varicocele ranges between 15% and 22% in male adults.…”
