Adolescent Cardiovascular Functional and Structural Outcomes of Growth Trajectories from Infancy: Prospective Community-Based Study

Abstract: We found little evidence that those with consistently higher BMI from infancy experienced poorer cardiovascular function or structure by the age of 14 years. Nonetheless, childhood BMI trajectories track strongly, meaning resolution of risky BMI, while essential to decrease later cardiovascular risk, is unlikely without intervention.

“…Almost half (45%) of the adolescents in this study had a classification of overweight/obese, consistent with the national average among African-American adolescents (1). BMI trajectories during childhood are strongly associated with cardiovascular risks, above and beyond BMI at any given point in time in childhood (2). The mid-teen years may "represent a 'tipping point' after which the vascular system no longer adapts to childhood adiposity and readily measurable vascular damage begins to occur."…”

“…BMI trajectories during childhood are strongly associated with cardiovascular risks, above and beyond BMI at any given point in time in childhood . The mid‐teen years may “represent a ‘tipping point’ after which the vascular system no longer adapts to childhood adiposity and readily measurable vascular damage begins to occur.” Adolescence is characterized by relative immaturity of the frontally mediated cognitive control system which is vital to the inhibition of impulsive responses and to decision‐making based on environmental stimuli . Underdeveloped inhibitory skills combined with greater autonomy over food choices in adolescence pose a risk for weight gain during this critical time.…”

“…Recent studies have looked at trajectory groups of BMI with different numbers and profiles of trajectories over childhood including early childhood 1,8 and later childhood through to adolescence. 25,26 Two studies have followed a cohort from birth to age 14 years with focus on 1 outcome [27][28][29] as distinct from our study of a wide number of cardiometabolic markers.…”
