Introduction Despite the implementation of complex interventions, ICU mortality remains high and more so in developing countries. The demand for critical care in Sub-Saharan Africa is more than ever before as the region experiences a double burden of rising rates of non-communicable diseases (NCD) in the background battle of combating infectious diseases. Limited studies in Tanzania have reported varying factors associated with markedly high rates of ICU mortality. Investigating the burden of ICU care remains crucial in providing insights into the effectiveness and challenges of critical care delivery. Material and Methods A single-center retrospective study that reviewed records of all medically admitted patients admitted to the ICU of the Aga Khan Hospital, Dar-es-Salaam, from 1st October 2018 to 30th April 2023. To define the population in the study, we used descriptive statistics. Patients’ outcomes were categorized based on ICU survival. Binary logistic regression was run (at 95% CI and p -value < 0.05) to identify the determinants for ICU mortality. Results Medical records of 717 patients were reviewed. The cohort was male (n=472,65.8%) and African predominant (n=471,65.7%) with a median age of 58 years (IQR 45.0–71.0). 17.9% of patients did not survive. The highest mortality was noted amongst patients with septic shock (29.3%). The lowest survival was noted amongst patients requiring three organ support (n=12,2.1%). Advanced age (OR 1.02,CI 1.00–1.04), having more than three underlying comorbidities (OR 2.50,CI 1.96–6.60), use of inotropic support (OR 3.58,CI 1.89–6.80) and mechanical ventilation (OR 9.11,CI 4.72–18.11) showed association with increased risk for mortality in ICU. Conclusion The study indicated a much lower ICU mortality rate compared to similar studies conducted in other parts of Sub-Saharan Africa. Advanced age, underlying multiple comorbidities and organ support were associated with ICU mortality. Large multi-center studies are needed to highlight the true burden of critical care illness in Tanzania.

BackgroundNew intensive care unit (ICU) nurses often experience stress because of concerns about potentially harming their patients in a work environment that demands the rapid development of several skills in a limited training period.AimThis study aimed to investigate the prioritisation of educational needs within adult ICUs, focusing on how new nurses evaluate their current knowledge and perceive the most critical competencies.Study DesignA cross‐sectional study was conducted among a convenience sample of 102 new ICU nurses in general and tertiary hospitals in South Korea. Educational needs were assessed using a structured questionnaire for new ICU nurses. This study investigated educational needs using paired t‐tests, Borich's assessment model and the Locus for Focus model.ResultsOnly 48% of participants were satisfied with their education. The highest‐rated educational content included preparing to use a defibrillator (95% CI = 2.44–3.28, p < .001), administering emergency drugs for cardiopulmonary resuscitation (CPR) (95% CI = 2.09–2.91, p < .001), starting and maintaining continuous renal replacement therapy (95% CI = 1.50–2.42, p < .001), applying and maintaining a ventilator (95% CI = 1.42–2.08, p < .001), preparing for intubation (95% CI = 1.23–1.97, p < .001), reporting to the emergency team, preparing equipment for CPR (95% CI = 1.12–1.94, p < .001) and drug calculation (95% CI = 0.87–1.53, p < .001).ConclusionsThese findings indicate that educational programmes for new ICU nurses should be developed considering the aforementioned priorities. Furthermore, nurse educators should adopt a practical and active instructional method to repeatedly clarify content, prioritising the improvement of knowledge and performance of new ICU nurses.Relevance to Clinical PracticeThis study guides clinical educators and managers in focusing on areas where new ICU nurses need additional training. Effective nurse residency programmes tailored to the specific needs of new ICU nurses can enhance their confidence and ability to handle ICU nursing challenges.

BACKGROUND Intensive care is a crucial component of hospital care, reserved for patients with potentially recoverable conditions who can benefit from invasive treatment. However, in developing countries like Nepal, resources and availability of intensive care become hurdles in delivering healthcare. This study presents the pattern of admission and outcome of patients' management in a general intensive care unit (ICU) of this tertiary care center in Lumbini Province, Nepal. METHODS A cross sectional study conducted in the Department of Internal Medicine, Bheri Hospital between July 2021 to June 2022. A total of 792 patient’s details were obtained from the record section. Demographic data, diagnosis, duration of stay in ICU, managing units, and outcome were collected and analyzed with Statistical Package for the Social Sciences 20. RESULTS A total of 729 patients were admitted in the study period; 57.82% were male and 42.18% were female. Majority 60% of admissions were from Department of Medicine followed by Cardiology 24% and Surgery 10%. Pneumonia and Chronic Obstructive Pulmonary Disease 18.30% followed by Myocardial Infraction 14.52%, Acute abdomen 10.22% and sepsis 9.84% were the most common morbidity. Mean duration of ICU stay was 3.5. days. Discharge rate was 69.82% and the mortality rate was 17.80 %. CONCLUSION The most common patients admitted in the ICU of Bheri hospital were mostly Medical cases. Surgical and obstetric cases were lesser in number. The outcomes were comparable with other multidisciplinary ICU of similar settings.
