Administration of increasing doses of gonadotropin-releasing hormone in men with spinal cord injury to investigate dysfunction of the hypothalamic–pituitary–gonadal axis

Abstract: Study Design Prospective Objectives To determine the optimum gonadotropin releasing hormone (GnRH) dose to identify dysfunction of the hypothalamic-pituitary-gonadal axis in men with spinal cord injury (SCI). Setting Metropolitan Area Hospitals, New York and New Jersey, USA Methods SCI men [16 hypogonadal (HG=serum testosterone <12.1 nmol/L) & 14 eugonadal (EG)] and able-bodied (AB) men (27 HG & 11 EG) were studied. GnRH (10, 50, 100 μg) was randomly administered intravenously on three separate visits. B… Show more

“…The authors demonstrated a 0.6%/year decline in serum total testosterone compared to a 0.4%/year decline in men in the Massachusetts Male Aging Study. In addition, low serum testosterone levels were observed at earlier decades of life with a higher prevalence in men with SCI than in non-disabled control subjects [ 49 , 50 ]. The role of estrogens in bone homeostasis is well-recognized, as the decrease in estrogen at menopause in women is the primary cause of osteoporosis.…”

“…After SCI, 45–60% of men exhibit low T (hypogonadism, T < 300 ng/dL) [ 91 , 92 , 93 ] that results, in-part, from secondary testicular dysfunction [ 94 ] and impairments in the hypothalamic-pituitary axis [ 95 , 96 ]. Hypogonadism is ~four times more prevalent after SCI than in able-bodied men [ 7 ], with T concentrations remaining >25% below that of age-matched healthy men for an extended duration after SCI [ 91 , 97 ].…”

“…Subsequent legislative action of criminalizing marijuana possession in the Boggs act in 1951 which introduced mandatory minimum sentencing for cannabis possession [20,21]. Cannabis, having been used for over 10,000 years, became a criminal activity in the United States < 70 years ago, based on absolutely no evidence [22]. The final political victory was found in the Single Convention on Narcotic Drugs 1961 and 1972 [23].…”
