Objective To develop expert consensus recommendations regarding radiation therapy for gynecologic malignancies during the COVID-19 pandemic. Methods An international committee of ten experts in gynecologic radiation oncology convened to provide consensus recommendations for patients with gynecologic malignancies referred for radiation therapy. Treatment priority groups were established. A review of the relevant literature was performed and different clinical scenarios were categorized into three priority groups. For each stage and clinical scenario in cervical, endometrial, vulvar, vaginal and ovarian cancer, specific recommendations regarding dose, technique, and timing were provided by the panel. Results Expert review and discussion generated consensus recommendations to guide radiation oncologists treating gynecologic malignancies during the COVID-19 pandemic. Priority scales for cervical, endometrial, vulvar, vaginal, and ovarian cancers are presented. Both radical and palliative treatments are discussed. Management of COVID-19 positive patients is considered. Hypofractionated radiation therapy should be used when feasible and recommendations regarding radiation dose, timing, and technique have been provided for external beam and brachytherapy treatments. Concurrent chemotherapy may be limited in some countries, and consideration of radiation alone is recommended. Conclusions The expert consensus recommendations provide guidance for delivering radiation therapy during the COVID-19 pandemic. Specific recommendations have been provided for common clinical scenarios encountered in gynecologic radiation oncology with a focus on strategies to reduce patient and staff exposure to COVID-19.

Aim: Evaluate outcome of Concomitant Hypofractionated Simultaneous Integrated Boost (SIB) Intensity-Modulated Radiotherapy (IMRT) Plus Concurrent Weekly Cisplatin monotherapy for Newly Diagnosed Locally Advanced Cancer Cervix with Positive LNs in elderly patients. Patients and Methods: Twenty-one patients were included diagnosed with locally advanced cancer cervix ≥ 65 years old with good performance status and adequate organ functions. Eligible patients received concomitant chemotherapy in form of cisplatin monotherapy (40 mg/m 2 ) weekly, during radiotherapy treatment in form of External Beam Radiotherapy (EBRT) with SIB due to patients` refusal or inability to receive brachytherapy (BRT). Treatment toxicity, progression free survival (PFS) and overall survival (OS) were recorded and analyzed. Results: The median age reported was 70 years, (range: 65-76 years) and the median follow up period was 35 months (ranged from 18-47 months). The OS was 90.2% at 2 years and 76.8% at 3 years. The median time to progression was 27 months (ranged from11 -46 months). The PFS was 75.6 % at 2 years and 63.5% at 3 years. Conclusion: In locally advanced cervical cancer, SIB-IMRT could be considered an effective and safe treatment alternative in elderly patients or unavailable brachytherapy.

Background: To evaluate the feasibility and clinical preliminary results of weekly cisplatin and volumetric-modulated arc therapy to the pelvis with simultaneous integrated boost to macroscopic disease in a cohort of elderly patients. Materials and Methods: Inclusion criteria of this prospective study were age !70 years, Karnofsky performance status 70 to 100, locally advanced histologically proven squamous cervical carcinoma, and patients unable to undergo brachytherapy. Radiation doses prescribed were 66 Gy to the macroscopic disease and 54 Gy to the pelvic nodes in 30 fractions. Weekly cisplatin dose was 40 mg/mq. Results: A total of 30 patients were recruited. Median follow-up was 32 months (range: 8-48 months). Median age was 72 years (range: 70-84 years). The 3-year overall survival and local control were 93% and 80%, respectively. The median time to progression was 24 months (range: 6-30 months). Analyzing clinical outcome grouping based on the stage of disease, II versus III, the 3-year overall survival was 100% and 85%, respectively. The 3-year local control was 91% for stage II and 67% for stage III. Acute and late toxicities were acceptable without severe events. Conclusion: Weekly cisplatin and volumetric-modulated arc therapy-simultaneous integrated boost for radical treatment of advanced cervical cancer in the current cohort of elderly patients were feasible. Long-term results and prospective randomized trials are advocated.
