Adjustable Gastric Banding as a Revisional Bariatric Procedure after Failed Gastric Bypass

Abstract: These results indicate that the addition of an ASGB causes significant weight loss in patients with poor weight loss outcome after RYGBP. The fact that no anastomosis or change in absorption is required may make this an attractive revisional strategy. Long- term evaluation in a larger population is warranted.

“…Restrictive procedures such as LAGB have well-documented safety and weight loss efficacy [21]; and while RYGBP has been shown to effectively treat morbid obesity, Angrisani et al reported the need for reoperation in 12.5% of patients to treat potentially lethal complications [5]. Bessler et al previously examined the use of LAGB as a rescue procedure for failed RYGBP [22]. Patients lost an average of 38.1 ± 10.4% and 44.0 ± 36.3% excess weight at 12 and 24 months follow-up, respectively, after placement of an adjustable silicone gastric band around the gastric pouch in eight patients who had a previously failed RYGBP.…”

“…Finally, in cases of failed BPD ? DS, some have advocated the use of a pouch reduction procedure [236], and in cases of failed RGB, either AGB to improve the restrictive component [237] or lengthening to improve the malabsorptive component [238] have been used. Comparative data are lacking.…”

“…Likewise, most authors advocate RGB for revision of AGB because of complications or insufficient weight loss [217][218][219], although other operations have been applied [234,235]. Finally, in cases of failed BPD+DS some have advocated use of a pouch reduction procedure [236], and in failed RGB use of either AGB to improve the restrictive component [237] or lengthening to improve the malabsorptive component [238]. Comparative data are lacking.…”
