Adjunctive supragingival irrigation with acetylsalicyclic acid in periodontal supportive therapy

Abstract: To assess the clinical efficacy of adjunctive supragingival irrigation with buffered 0.3% acetylsalicylic acid (ASA), 60 patients with periodontitis receiving supportive periodontal therapy were randomly assigned to 1 of 3 home regimens: (1) 1x daily adjunctive supragingival irrigation with 300 ml water immediately followed by 200 ml of buffered 0.3% ASA; (2) 1x daily adjunctive supragingival irrigation with 500 ml water; or (3) normal oral hygiene alone. Clinical parameters were assessed at baseline and 6 mon… Show more

“…The effects of irrigation with different medicaments and with water on gingival and periodontal health have been assessed in many studies, and results found showed consistent benefits in healthy subjects (Cobb et al 1988, Flemmig et al 1995, Cutler et al 2000). Electron microscopic studies have shown evidence of bacterial cell disruption following water irrigation (Brady et al 1973).…”

“…Clinical efficacy of irrigation with water alone was compared to the irrigation with water and buffered 0.3% acetylsalicylic acid, and normal oral hygiene alone (control group). Result showed that supragingival irrigation with water alone or in combination with buffered 0.3% acetylsalicylic acid significantly improved GI and PPD scores when compared to regular oral hygiene alone (Flemmig et al 1995). It was also noted in the same study that supragingival irrigation with water alone resulted in a significant reduction in BOP when compared to both irrigation with buffered 0.3% acetylsalicylic acid and with control groups.…”

“…Thus, regular supragingival irrigation with either 0.3% ASA or water besides usual oral hygiene seems to be a valuable extra periodontal supportive remedy in patients showing the signs of moderate to severe periodontitis. [20] Antiseptic irrigations have been employed for managing plaque buildup. [21] Peridex, Perioguard and Listrine are the antiseptic irrigations clinically approved by American Dental Association.…”

“…These observations suggest that much of the clinically visible gingival inflammation is associated with neutrophil LTB 4 synthesis that would not be inhibited by most NSAIDs. This may explain the weak efficacy of NSAIDs in blocking clinical signs of gingival inflammation [57,58]. The NSAID ketoprofen, which is an inhibitor of both the cyclooxygenase and lipoxygenase pathways, may be a notable exception [59].…”
