Adjacent Segment Disease after Lumbar or Lumbosacral Fusion: Review of the Literature

Abstract: Biomechanical alterations likely play a primary role in causing adjacent segment disease. Radiographically apparent, asymptomatic adjacent segment disease is common but does not correlate with functional outcomes. Potentially modifiable risk factors for the development of adjacent segment disease include fusion without instrumentation, protecting the facet joint of the adjacent segment during placement of pedicle screws, fusion length, and sagittal balance. Surgical management, when indicated, consists of decom… Show more

“…Adjacent segment degeneration is a common long-term sequela or complication of spinal fusion surgery. The exact aetiology is uncertain but alterations in facet loading, hypermobility, and increased intradiscal pressure at the segments adjacent to fusion mass is believed to play a key role [15,21,35,44,51,60]. Superior segment facet violation or laminectomy has recently shown in vitro to destabilize the adjacent level in transpedicular fixation [9].…”

“…However, discordant findings were found using this treatment, as it did not always result in clinical improvement (7)(8)(9)(10)(11)(12). In fact a high incidence of accelerated disc degeneration at adjacent levels has been reported when patients were treated with a rigid instrumentation system (13)(14)(15).…”

“…The clinical relevance of ASD is low in the great majority of cases [2,[6][7][8]. Cauda equina syndrome is a very unusual onset of ASD.…”

“…Adjacent segment degeneration (ASD) has been defined as a recurrence of symptoms associated with the degeneration at the free segment above a fusion after a symptom-free period [5]. The incidence of (symptomatic) ASD is low, ranging from 5.2 to 18% [2]. We report an unusual case of a patient developing cauda equina syndrome due to recurrent ASD, 8 years after his last posterior instrumented spinal fusion.…”
