Adiposity, Inflammation, and Risk for Death in Black and White Men and Women in the United States: The Reasons for Geographic and Racial Differences in Stroke (REGARDS) Study

Abstract: Among middle-aged and older adults, the addition of CRP was most informative among underweight and normal-weight individuals, not the obese. This negated our hypothesis that increased levels of CRP would differentiate individuals at higher mortality risk due to excess adiposity.

“…However, subgroup analyses examining these relationships in obese compared with nonobese subjects were not performed (30). Likewise, in the REGARDS (Reasons for Geographic and Racial Differences in Stroke) cohort of Ͼ27,000 individuals, CRP levels Ͼ3 mg/l were most informative for all-cause mortality in underweight and normal-weight individuals, rather than the severely obese (p interaction ϭ 0.01, white subjects) (31).…”

“…Serum insulin was measured by electrochemiluminescence immunoassay by using the Roche Elecsys 2010 system (Roche Diagnostics). Serum highsensitivity C-reactive protein (hs-CRP) was analyzed by particle-enhanced immunonephelometry by using the BNII nephelometer (N High Sensitivity CRP; Dade Behring) (33).…”

“…Figure 2). Indeed, contemporary estimates from other sources suggest that the cardiometabolic risk [48] and mortality risk [49] for US black women begin to rise significantly only above ,33 kg/m 2 . Assessing black women's risk by the p75 WC threshold instead of the BMI threshold might better clarify their true risk.…”
