Adiposity and risk of ischaemic and haemorrhagic stroke in 0·5 million Chinese men and women: a prospective cohort study

Abstract: SummaryBackgroundChina has high stroke rates despite the population being relatively lean. Uncertainty persists about the relevance of adiposity to risk of stroke types. We aimed to assess the associations of adiposity with incidence of stroke types and effect mediation by blood pressure in Chinese men and women.MethodsThe China Kadoorie Biobank enrolled 512 891 adults aged 30–79 years from ten areas (five urban and five rural) during 2004–08. During a median 9 years (IQR 8–10) of follow-up, 32 448 strokes (ab… Show more

“…Cardiovascular disease (CVD) is a leading cause of death and disability worldwide and contributes to more than 17 million deaths annually [ 1 ], especially 8.4 million CVD deaths across Brazil, Russia, India, China, and South Africa (BRICS) in 2016 [ 2 ]. Obesity and its related metabolic disorders have been major risk factors for CVD globally, including in China [ 3 5 ]. However, people with obesity have variability in metabolic factors.…”

“…However, no relationship was found between obesity and hemorrhagic stroke within the normal BMI range (< 25 kg/m 2 ), nonetheless, a correlation between hemorrhagic stroke and below-normal BMI was indicated. As ischemic stroke concerns the majority of stroke cases, findings of intrace rebral hemorrhage should not detract from the fundamental importance of high obesity as the main modifiable determinant of all strokes [25].…”

“…However, a recent study demonstrated that obesity was significantly associated with increased incident risk of cardiovascular (CVD) and mortality from CVD in 10 prospective cohort studies among North Americans [31]. And the China Kadoorie Biobank (CKB) study found that every 5 kg/m 2 higher BMI was associated with 30% increased risk of IS among over 0.5 million normal Chinese adults [32]. The discrepancy among different studies may be due to the differences in study design, sample size, duration of follow-up, and age at recruitment.…”
