Adipose-Derived Stem Cells in Crohn's Rectovaginal Fistula

Abstract: Therapeutic options for recto-vaginal fistula in the setting of Crohn's disease are limited and many data are available in the literature. The manuscript describes the history of a patient who has been the pioneer of our Clinical Trials in treating this disease in fistulizing Crohn's disease environment. We believe it is the first time that a patient with this disease has been treated by adipose-derived stem cells in allogeneic form. The conclusion of our study with Mary is that the use of mesenchymal stem cel… Show more

“…The first one was published in 2003 [11] , and the last was published very recently, in 2013 [20] . Eight of these have been published by Spanish groups [11][12][13][14]17,[21][22][23] , two other papers come from South Korea [20,24] , and one from Italy [25] . The majority refer to ASC treatment of Crohn's disease-related fistulae.…”

“…Together with active research in the field of bone marrow-derived mesenchymal stem cells (BM-MSCs) and hematopoietic stem cells [16][17][18], autologous or allogenic adipose tissue-derived stem cells (ASCs) [14,[19][20][21][22][23] have been studied for management of Crohn's disease and other disorders. Of particular relevance to this study, ASCs could be considered to be safe and efficacious therapeutic tools for the treatment of Crohn's fistulae [20].…”

“…But now, after the reported paracrine effects in several MSCs therapies we know that MSCs, far from building those tissues, they exert immunomodulatory functions, secreting, in addition, several bioactive molecules that inhibit apoptosis and scarring at sites of injury, and stimulate angiogenesis and mitosis of tissue-specific progenitors (Caplan, 2009;Ito et al, 2010;Park et al, 2010). These actions have been found either when the implanted MSCs coming from BM or adipose derived in adherent cell cultures (García-Olmo et al, 2010) or when the mononuclear fraction of BM was infused. We cannot predict the extent of the paracrine effect, immunomodulatory, or if the effective replenishment of differentiated cells can be assigned, in each case, to the MSCs, or whether these effects have some degree of integration between them.…”
