Introduction: Vitamin D deficiency has been linked to the increased risk of several chronic diseases, especially in people living in the Northern Latitudes. The aim of this study was to assess the vitamin D status in older subjects born in 1945 in Northern Finland (latitude 65°North), and to examine its associations to components of metabolic syndrome (MetS). Methods: In this cross-sectional study, we invited 904 subjects born in 1945 from the Oulu region (Oulu45 cohort), out of an original cohort of 1332 subjects. In the cohort, plasma 25 hydroxyvitamin D (25OHD) levels were determined by an enzyme immunoassay of 263 men and 373 women, with a mean age baseline of 69±0.5 years old. We assessed the participants’ usage of vitamin D supplements, as well as their lifestyle factors, using a questionnaire. Results: Nearly 80% of the subjects had low vitamin D levels [either vitamin D deficient (<50 nmol/L) or insufficient (50 – 75 nmol/L)], and only 20% of the participants had sufficient vitamin D levels (>75 nmol/L) (based on the American Endocrine Society guidelines). The low vitamin D status was associated with a high prevalence of MetS; a significantly higher number of subjects with MetS (41%) had low vitamin D levels in comparison to the non-MetS subjects (38%) (p ≤ 0.05). The subjects under vitamin D supplementation had a significantly lower incidence of MetS (42.6% vs 57.4%) and its components in comparison to the non-supplemented subjects (p ≤ 0.05). Conclusions: Low vitamin D levels are a risk factor for MetS amongst other lifestyle factors, such as dietary habits and physical inactivity, among older subjects in the Northern Latitudes (65°North). Optimal supplementation of vitamin D, along with rich dietary sources of vitamin D, are highly recommended for older subjects as a means to positively affect, e.g., hypertension, insulin resistance, and obesity, as components of the MetS.

Cytokines are the signalling molecules that underlie inflammatory processes. Here, we performed genome-wide association study (GWAS) analyses of 47 circulating cytokines in up to 13,365 individuals to identify protein quantitative trait loci (pQTL). Applying a novel approach, we incorporated pQTL and expression quantitative trait loci (eQTL) data of 10,361 tissue samples in 635 individuals to identify biologically plausible genetic instruments to proxy the effect of cytokines. Using Mendelian randomization analysis, we explored the causal determinants of inflammatory cytokines, investigated inflammatory cascades and evaluated their effects on 20 diseases. We show evidence of body mass index (BMI), smoking and systolic blood pressure (SBP) being associated with inflammation, and specifically BMI affecting levels of active PAI-1, HGF, MCP1, sE-Selectin, sICAM1, TRAIL, IL6 and CRP. Our analysis highlights a key role of VEGF in influencing the levels of eight other inflammatory cytokines. Finally, we report evidence of sICAM affecting waist circumference and risk of major depressive disorder, evidence for TRAIL affecting the risk of cardiovascular diseases, breast and prostate cancer, and evidence for MIG affecting the risk of stroke. Overall, our results offer insight into inflammatory mediators of BMI, smoking and SBP, pleiotropic effects of VEGF, and circulating cytokines that increase the risk of cancer, cardiovascular, metabolic and neuropsychiatric diseases. All the studied cytokines represent pharmacological targets and therefore offer opportunities for clinical translation in diseases with inflammatory components.

Resumo: O objetivo do trabalho foi comparar biomarcadores em grupos de adolescentes classificados simultaneamente pelo índice de massa corporal (IMC) e percentual de gordura corporal, mensurados por meio da pletismografia por deslocamento de ar. Estudo transversal com 533 adolescentes de 18 e 19 anos de São Luís, Maranhão, Brasil. O IMC foi classificado em adequado (< 25kg/m2) e excesso de peso (≥ 25kg/m2). Definiu-se percentual de gordura corporal elevado ≥ 25% para o sexo masculino e ≥ o feminino. Os adolescentes foram classificados em quatro grupos: “eutrófico” (IMC e percentual de gordura corporal adequados), “obeso de peso normal” (IMC adequado com percentual de gordura corporal elevado), “excesso de peso com percentual de gordura corporal adequado” e “excesso de peso com percentual de gordura corporal elevado”. As meninas registraram maiores valores de “obesidade de peso normal” (15,6%) e “excesso de peso com percentual de gordura corporal elevado” (17,1%). Os adolescentes “obesos de peso normal” apesentaram maiores médias para colesterol total (172,5mg/dL) e LDL-colesterol (103,5mg/dL). Aqueles com “excesso de peso e percentual de gordura corporal elevado” registraram a menor média para HDL-colesterol (43,2mg/dL) em relação aos outros grupos; maior média para interleucina-6 (2,7pg/mL) em relação aos “eutróficos” e “excesso de peso e percentual de gordura corporal adequado”; e maiores medianas para triglicerídeos (114,0mg/dL) e proteína C reativa (0,14ng/mL) em relação aos “eutróficos” e “obesos de peso normal”. Os com “excesso de peso e percentual de gordura corporal adequado” apresentaram proteína C reativa igual aos com “excesso de peso e percentual de gordura corporal elevado”. A avaliação do estado nutricional pelo IMC é limitada, pois 6,8% dos adolescentes registraram percentual de gordura corporal elevado apesar do IMC normal, além disto, os “obesos de peso normal” apresentaram dois biomarcadores piores que os adolescentes com “excesso de peso e percentual de gordura corporal elevado”.
