Adherence to Guidelines for Avoiding Drug Interactions Associated with Warfarin - A Nationwide Swedish Register Study

Abstract: PurposeTo investigate the extent to which clinicians avoid well-established drug-drug interactions associated with warfarin. We hypothesised that clinicians would avoid combining non-steroidal anti-inflammatory drugs (NSAIDs), tramadol and sulfamethoxazole with warfarin.MethodsA cross-sectional analysis of nationwide dispensing data was performed in Swedish individuals 18 years or older (n =  7 563 649). Odds ratios of interacting NSAIDs, tramadol and sulfamethoxazole versus respective prevalence of comparator… Show more

“…The mechanism through which tramadol may increase warfarin response is not fully understood, but it is suspected to arise from inhibition of serotonin re‐uptake in platelets, while the liability to an interaction may be related to the CYP2D6 activity . A recent Swedish study illustrated that physicians refrain from prescribing NSAIDs during warfarin therapy while tramadol is rarely avoided . Our study supports these findings by demonstrating a decrease in the frequency of NSAID purchasing after warfarin initiation.…”

“…NSAIDs have been among the most common interacting drugs in inpatients and outpatients taking warfarin [3,4,9,27]. This trend seems to have changed and similar results to ours have been recently published in large register-based studies [11,28,29]. These findings may indicate increased awareness of clinicians of the increased bleeding risk associated with concomitant use of NSAIDs with warfarin.…”

“…We thought that we must increase the TTR values of patients on warfarin therapy in Turkey at least by educating patients and physicians. Lindh et al (20) showed that Swedish doctors in the vast majority of cases refrain from prescribing NSAIDs to patients already on warfarin. There was no study showing the approach of Turkish physicians with respect to the patients on warfarin.…”
