PURPOSE Oral anticancer medications (OAMs) improve treatment outcomes and survival in women with breast cancer (BC). However, adherence to OAM therapy remains suboptimal. This scoping review provides evidence of adherence to OAMs among African women with BC. METHODS We searched four databases and gray literature, using guidance from the Joanna Briggs Institute. Thirteen studies on adherence rates, determinants, and interventions were included. NVivo 12 software was used to perform thematic analysis of the included studies. The determinants (barriers and facilitators) associated with adherence were analyzed according to the five dimensions of the WHO multidimensional adherence model. RESULTS Most studies (n = 11, 85%) focused on endocrine medication. Depending on the definition, measurements, and assessment period, the nonadherence rates ranged from 4.3% to 65.4% for endocrine medications, 80.9% for cytotoxic chemotherapies, and 32.7% for combined medications. The significant barriers associated with adherence include Islamic religion, concurrent comorbidities, mastectomy, anastrozole treatment, side effects, unawareness of treatment insurance coverage, and seeking treatment from traditional healers. Thorough therapeutic communication regarding treatment, neoadjuvant chemotherapy, and adequate social support significantly facilitate adherence. A randomized controlled trial of breast nursing interventions reported improved patient adherence. CONCLUSION The evidence mapped from studies that evaluated OAM adherence in women with BC indicates that nonadherence to OAMs is common. Applying context-specific standardized measures to assess adherence and facilitators or strategies targeting the identified barriers can optimize adherence and treatment outcomes. Effective interventions to improve adherence are limited. Therefore, further empirical and interventional studies in Africa are required to enhance the evidence.

Background Cancer is the leading cause of death worldwide, and poor adherence to chemotherapy has become a public health issue in developing countries, including Ethiopia. Objective To assess chemotherapy adherence and associated factors among patients with cancer in the Amhara Region, Northeast Ethiopia. Methods A hospital-based prospective cross-sectional study design was employed, and a total of 460 cancer patients were enrolled using a systematic random sampling method at Felege Hiwot and Dessie comprehensive specialized hospitals from May 15, 2022, to July 15, 2022, in the Amhara region. The data was gathered through a review of medical records and a face-to-face interview and entered into Epi-Data version 4.6 before being exported to SPSS version 26 for analysis. Furthermore, at 95% confident interval (CI), multivariable binary logistic regression was used, and variables with P-values of <.05 were found to be significant. Result The overall response rate in this study was 94.1% (433), with 42.3% of patients adhering to chemotherapy. Moreover, having a family history of cancer [AOR = 3.58, 95% CI (2.22, 5.76)], being female [Adjusted Odds ratio (AOR) = 2.17, 95% CI: (1.31, 3.60)], having no history of comorbidity [AOR = 2.74, 95% CI (1.56, 4.81)], having side effects from chemotherapy [AOR = 3.50, 95% CI (1.55, 7.90)], and having social support [AOR = 1.52, 95% CI (1.21, 1.95)] were important predictors of chemotherapy adherence. Conclusion and recommendations: Patients' understanding of illness characteristics and treatment should be improved through health education. Involving family members in the treatment plan will also improve adherence to chemotherapy in this context.

Background: Chemotherapy compliance is a joint effort and silent understanding between health workers and clients. Inadequate drug compliance is concerning due to the decreased treatment outcome of the drug. This will lead to poor survival, increased relapse, treatment failure, and elevated healthcare costs. A measure of patient treatment adherence is important for attracting the attention of healthcare providers to achieve optimal outcomes such as curative treatment or quality of life improvement. Objective: To assess the adherence to chemical treatment and associated factors among cancer patients in the Amhara Region, Northeast Ethiopia, in 2022. Methods and Materials: A retrospective cohort study was conducted with 460 cancer patients in the Amhara region using systematic randomized sampling from May 28, 2022, to June 22, 2022. An interview-based, previously tested questionnaire was used for data collection. The data have been entered into Epi-Data version 4.6 and subsequently exported to Statistical Package for Social Science version 26 for analysis. A bivariable logistic regression was carried out, and variables with a p value of 0.25 were analyzed to select the candidate variable for multivariable logistic regression. A multivariable logistic regression was carried out, and variables with a p value less than 0.05 were considered important factors and are presented as the AOR and 95% CI. The suitability of the model was verified with the Hosmer Lemeshow test, and multicollinearity was verified with a standard error. Results: Of the total respondents, 94.1% (433) of the cancer patients were involved, and the proportion of patients who adhered to chemotherapy was 42.3%. Having a family history of cancer [AOR = 3.58, 95% CI (2.22, 5.76)], being a woman [AOR = 2.17, 95% CI: (1.31, 3.60)], not having a history of comorbidity [AOR = 2.74, 95% CI (1.56, 4.81)], having no serious side effects from chemotherapy[AOR = 3.50, 95% CI (1.55, 7.90)], or social support [AOR = 1.52, 95% CI (1.21, 1.95)] were associated factors with chemotherapy adherence. Conclusion and recommendations: Health workers should contribute to the development of social support and mechanisms to reduce the occurrence of side effects of chemotherapy medication among cancer patients who have comorbidities.
