Adenoma with gastric differentiation (so-called pyloric gland adenoma) in a heterotopic gastric corpus mucosa in the rectum

Abstract: In a 46-year-old man, a pedunculated rectal polyp measuring 3.0x3.0x2.0 cm was diagnosed histologically as a pyloric gland-type adenoma arising in heterotopic gastric corpus mucosa. The luminal site was covered by glands of the gastric foveolar type, displaying focal marked proliferation interpreted as low-grade intraepithelial neoplasia. A bidirectional gastric differentiation was found: most lower glandular structures showed positivity for the deep gastric mucin core protein Muc 6 and superficial positivity … Show more

“…In 2003, we first reported systematic clinicopathological analyses of pyloric gland adenoma, in English language and Japanese journals [38,39]. Since then, there have been reported cases of pyloric gland adenoma arising in the proper esophagus [40], Barrett's esophagus [41], and in gastric heterotopia of rectum [42].…”

“…1 d), leading to a final diagnosis of high-grade pyloric gland adenoma originating from heterotopic gastric mucosa within the duodenal bulb. The present case illustrates an extragastric pyloric gland adenoma originating from heterotopic gastric mucosa, as has so far been documented in only two other duodenal lesions [1,3] and one rectal lesion [5]. Gastric heterotopia, however, appears to be the prerequisite for tumor histogenesis.…”

“…The exact mechanism for the development of heterotopic gastric mucosa in the rectum remains to be seen. Several theories including developmental errors or prior inflammatory reactions have been postulated [3,5] . The majority of the reported patients were symptomatic, and complete relief could be obtained following treatment including surgical resection, endoscopic polypectomy or ablation [6] .…”

“…In our case the biopsy from the rectal polyp exhibited foveolar hyperplasia, and the biopsy from the whitish discolored area demonstrated heterotopic fundic and pyloric-type gastric mucosa, showing clear correlation between endoscopic features and histopathologic findings. There have been previous reports of Helicobacter pylori infection and the development of adenoma in heterotopic gastric mucosa of the rectum [5,7] .…”
