Adenoid cystic carcinoma of the breast metastatic to the kidney. A clinically symptomatic lesion requiring surgical management

Abstract: Adenoid cystic carcinoma (ACC) of the breast is a rare histologic type of breast cancer associated with a good prognosis. A woman who earlier had a pulmonary metastasis surgically resected 6 years postmastectomy developed clinically symptomatic bleeding from a renal metastasis 12 years postmastectomy. An atypical angiographic picture (moderate vascularization) for secondary renal neoplasms was observed. The characteristic histologic features of ACC were present in all specimens; ultrastructural characteristics… Show more

“…[1][2][3][4][5][6][7] Most adenoid cystic carcinomas follow a favorable clinical course with a low prevalence of axillary lymph node metastases (approximately 10%). Tumors showing a predominant solid pattern of growth however reportedly have a more aggressive behavior, 8 with distant metastases despite the absence of lymph node involvement and poor 5-year overall survival. 6,[9][10][11][12][13] Adenoid cystic carcinoma, especially when showing a predominant tubular-cribriform growth pattern, shares many histological features with infiltrating cribriform and tubular carcinoma.…”

“…[3,4] Herzberg et al reported metastatic adenoid cystic carcinoma kidney after 12 years of mastectomy for adenoid cystic carcinoma breast. [3] Awakura et al diagnosed metastatic adenoid cystic carcinoma kidney 5 years after parotidectomy for adenoid cystic carcinoma. Four years after left renal nephrectomy, [4] multiple metastases were demonstrated in the other kidney, liver, lungs and brain by CT scan.…”

“…[1][2][3][4][5][6][7][8] It is a slowgrowing, but aggressive neoplasm with a remarkable capacity for recurrence. [9,10] It can arise in any salivary gland site, but approximately 50-60% develops within the minor salivary glands.…”

“…4 Furthermore, renal medullary carcinoma and mucinous tubular and spindle cell carcinoma entity that exhibits significantly worse outcome than luminal type carcinomas. 6,8 It is then paradoxical that ACC of the breast which is immunophenotypically classified as basal cell type cancer, have been particularly associated with favorable outcome.…”

“…Only several cases with high grade tumors and distant metastases have been published, and only one case with symptomatic metastasis to the kidney has previously been reported. 4 We report here a case of ACC of the breast with symptomatic renal metastasis, and for the first time show coexistent mutations in genes encoding phosphoinositide-3-kinase catalytic alpha subunit (PIK3CA) and phosphatase and tensin homolog (PTEN). Both PIK3CA and PTEN are integral part of the mammalian target of rapamycin (mTOR) signaling pathway.…”
