ÖZETGiriş: İnce bağırsağın malign tümörleri tüm gastrointestinal sistem malignitelerinin %2-3' ünü oluşturmaktadır. İnce bağırsağın primer malignitelerinin %95' ini adenokarsinomlar oluşturmaktadır. İleri yaşlarda sıklığı artmaktadır. Sıklıkla yerleşim yeri distal duodenum ve proksimal jejunumdur. Tanı genellikle rastlantısal olarak konulmaktadır. Cerrahi en etkin tedavi yöntemidir. Vaka Sunumu: Bu yazıda, akut strangüle herni sebebiyle acil operasyona alınmış ve operasyon esnasında ileumda tespit edilen tümöral kitle ile ileum adenokarsinomu tanısı konulan 67 yaşında kadın hasta sunulmuştur. Sonuç: İnce bağırsak adenokanserlerinde en etkin tedavi yöntemi cerrahidir. Anahtar kelimeler: İnce bağırsak, adenokanser, operasyon ABSTRACT Introduction: Malignant tumors of the small intestine comprise 2-3% of all gastrointestinal cancers. Adenocarcinomas account for 95% of primary small intestine malignancies. The prevalence of adenocarcinoma increases with advanced age. They are commonly located in the distal duodenum and the proximal jejunum. They are usually diagnosed coincidentally and are most effectively treated by surgery. Case Presentation: Here we present a 67-year-old female patient who underwent emergency surgery for acute strangulated hernia during which a tumoral mass was discovered in the ileum, leading to a diagnosis of adenocarcinoma of the ileum. Conclusion:The most effectively treated method is surgery in small intestine adenocarcinomas.

IntroductionPrimary small bowel malignancy is unusual and accounts for 1–3% of all gastrointestinal tract neoplasms. Adenocarcinoma is one of the most common histologic types, but its frequency decreases with more distal locations. Its clinical presentation is nonspecific and is usually associated with advanced disease, which contributes to delayed diagnosis.Presentation of caseA 66-year-old woman was admitted to the hospital with a 6-day history of progressively worsening abdominal pain localized in the right lower quadrant, nausea, and vomiting. Investigation revealed an inflammatory appendiceal tumor. The patient underwent surgery and an unexpected tumor involving the distal ileal segment and ileocecal appendix was found. Right radical hemicolectomy with en bloc resection of the distal ileum was performed. Histopathological examination revealed adenocarcinoma of the ileum.DiscussionThis rare entity is associated with a nonspecific clinical presentation that contributes to delayed diagnosis and treatment, and consequently to a worse prognosis. Approximately half of the cases are only diagnosed at surgery. Primary treatment consists of wide resection with locoregional lymphadenectomy. The role of adjuvant chemotherapy has yet to be determined.ConclusionThis case demonstrates an unusual condition characterized by late and challenging diagnosis. We highlight the importance of an earlier diagnosis and optimal treatment for improved patient outcomes.

Bevacizumab in metastatic small-bowel adenocarcinoma: A systematic review and meta-analysis

Cancers of the small bowel could account for less than 5% of all gastrointestinal malignancies. Of these tumors, adenocarcinomas were the major histologic subtype and generally carried a poor prognosis. High expression of vascular epithelial growth factor (VEGF) could be seen in small bowel adenocarcinomas. A systematic review was conducted here to determine if bevacizumab, a recombinant humanized antibody against VEGF, could offer clinical benefit among patients with metastatic small bowel adenocarcinoma when combined with chemotherapy. A search for relevant published and unpublished studies was performed using PubMed, ScienceDirect, Google Scholar, the American Society of Clinical Oncology meetings library, ClinicalTrials.gov, and ISRCTN registry. Information on study design, methods, intervention, and outcomes were extracted from selected eligible studies. Methodological quality was then assessed using the Newcastle-Ottawa Scale. There was a significant improvement in mean overall survival with the addition of bevacizumab with chemotherapy versus chemotherapy alone. The use of bevacizumab with chemotherapy, likewise improved progression-free survival and objective response rate compared to chemotherapy alone. Continued use of bevacizumab beyond first progression also appeared to show benefit. The conduct of prospective controlled studies by consortia to offset the rarity of small bowel adenocarcinomas could further elucidate the efficacy of bevacizumab in the treatment of this disease.
