Adenocarcinoma of the Scrotum (Extramammary Paget’s Disease): Case Report and Review of the Literature

Abstract: A case is reported of extramammary Paget's disease of the scrotum, with metastases to the superficial femoral lymph nodes. Extramammary Paget's disease and its urological spectrum are reviewed, and the management is discussed. Treatment consisted of surgery and irradiation therapy. The patient appears to be free of disease, despite metastatic nodal involvement, 2 1/2 years after diagnosis.

“…The lesions start on the scrotum, the penis or the inguinal folds and may spread to the abdomen; rarely, they are limited to the glans penis. Inguinal lymph node enlargement may be present, 60 occasionally associated with oedema of the legs, and may be inaugural. Association with an underlying carcinoma (of the prostate, bladder, testicles, ureter, kidney) occurs in 11% of cases 61 …”

“…EMPD generally presents as a 'resistant' eczematous lesion in patients between 50 and 80 years of age. 5 Histopathologically, EMPD is characterized by Paget's cells, which have abundant pale-staining cytoplasm and large atypical nuclei, appearing singly or in clusters. 3 Areas with a high density of apocrine glands, namely the penoscrotal area in men and the vulva in women are sites of predilection.…”

“…3,4 Local control and prognosis of patients with EMPD are related to the presence of adnexal and visceral malignancy as well as to the extent of metastatic spread. 5 Histopathologically, EMPD is characterized by Paget's cells, which have abundant pale-staining cytoplasm and large atypical nuclei, appearing singly or in clusters. Treatment of EMPD is sometimes difficult because of its tendency to be multifocal.…”
