Adenocarcinoma of the Renal Pelvis

Abstract: Two cases of intestinal-type adenocarcinoma of the renal pelvis (ARP) in 48-and 63-year-old females are presented. Both had a previous history of urinary obstructive disease, one secondary to staghorn calculus and the other to postirradiation stenosis following treatment for carcinoma of the cervix. Immuno-histochemical findings revealed marked positivity to epithelial membrane antigen and focal positivity to cytokeratins and carcinoembryonic antigen. Chronic irritation of the renal pelvis plays an important r… Show more

“…Few reports of the synchronous presentation of renal cell carcinoma (RCC) or transitional cell carcinoma (TCC) and renal stones have been published. RCC seems to be more frequently seen incidentally than TCC according to the literature [3][4][5][6] . Woodhouse [7] and Devarajan et al [8] had a series of 5 cases in each of their reports.…”

“…Most tumors tend to be in the advanced stages and the prognosis is poor, with 60–70% of patients dying within 2–5 years after surgery 1 . In most reported cases the patients have a long history of irritant conditions, such as lithiasis, hydronephrosis and chronic infection 1–4 . The urothelium has a particular capacity for metaplastic responses to noxious stimuli.…”

“…The association is very rare and only one case has been previously reported 10 . Few reports have analyzed the histogenesis of pelvic adenocarcinomas 2,3 . We have further investigated the biology of adenocarcinomas of the renal pelvis using immunohistochemistry.…”

“…1 They are classified into 3 subtypes: nonintestinal papillar, tubulovillous, and mucinous adenocarcinoma. 1-3 The latter 2 categories are frequently associated with long-duration inflammation, urolithiasis, and hydronephrosis. 2,3 Although their pathogenesis is not well defined, adenocarcinomas probably develop from neoplastic transformation of the glandular cells of the urothelial intestinal metaplasia usually arising in response to chronic irritating stimuli.…”
