Added value of clinical prediction rules for bacteremia in hemodialysis patients: An external validation study

Abstract: Introduction Having developed a clinical prediction rule (CPR) for bacteremia among hemodialysis (HD) outpatients (BAC-HD score), we performed external validation. Materials & methods Data were collected on maintenance HD patients at two Japanese tertiary-care hospitals from January 2013 to December 2015. We enrolled 429 consecutive patients (aged ≥ 18 y) on maintenance HD who had had two sets of blood cultures drawn on admission to assess for bacteremia. We validated the predictive ability of the CPR us… Show more

“…We need to decide the timing of administration of therapy and choice of antimicrobial agents appropriately. Serum ALP levels have been reported as one example of a simple clinical prediction rule in the bacteraemia ‘BAC-HD score’ 43. In maintenance HD outpatients suspected of sepsis, elevated serum ALP levels may indicate the presence of bacteraemia and may aid in the decision to begin early antimicrobial therapy and in the choice of the antimicrobial agent.…”

“…The elevated incidence of septic arthritis in dialysis patients is likely to be the consequence of compromised cellular and humoral immunity and frequent exposure to urinary and vascular access, which increases the risk of bloodstream infection [ 9 ]. Compared to the general population, dialysis patients with bacteremia are susceptible to higher morbidity and mortality [ 10 ]. However, the prognostic factors for in-hospital outcomes in dialysis patients with septic arthritis have yet to be identified in the current literature.…”
