Tractor overturns are the most prominent contributing factor to death on U.S. farms and have been for decades. While public health researchers hypothesized years ago that overturn fatalities would largely disappear as older tractors were replaced by newer ones, this has not happened. In the meantime, farmers continue to die in these easily preventable and tragic events. Not only are tractor rollover protective structures (ROPS) 99% effective in preventing death in the event of an overturn, a national program exists to connect farmers with ROPS. While this program has been popular with farmers, and the cost-effectiveness of providing farmers with technical support and cost-sharing rebates has been demonstrated, many states lack access to ROPS rebates, even those states with the highest number of overturn fatalities. Given the evidence, it is difficult to reconcile the lack of policy or public investment in a solution that decisively addresses the most pernicious contributor to death in the agricultural industry. In this commentary, we highlight the science behind this proven solution to tractor overturns and the essential role that the media and implementation science can play in encouraging policy-makers to save lives.

ObjectivesTo evaluate an implementation programme of a community pharmacy medication review with follow-up (MRF) service using a hybrid effectiveness-implementation study design, and to compare the clinical and humanistic outcomes with those in a previously conducted cluster randomised controlled trial (cRCT).SettingCommunity pharmacies in Spain.Participants135 community pharmacies and 222 pharmacists providing MRF to polymedicated patients aged 65 or over.InterventionThe intervention was an implementation programme for the MRF service. A national level group was established, mirrored with a provincial level group. A series of interventions were defined (1) to engage pharmacy owners with the implementation model and (2) to provide training to pharmacists consisting of clinical case studies, process of MRF, communication skills and data collection methods and (3) practice change facilitators.Primary and secondary outcome measuresThe primary outcomes for the implementation programme were progress, reach, fidelity and integration. The secondary outcomes were number of medications, non-controlled health problems, emergency visits, hospitalisations and health-related quality of life, which were compared with a previous 6-month cluster RCT.Results55% of pharmacies reached the implementation phase and 35.6% remained in the testing phase at 12 months. A reach of 89.3% (n=844) was achieved. Fidelity average score was 8.45 (min: 6.2, max: 9.3) out of 10. The integration mean score was 3.39 (SD: 0.72) out of 5. MRF service outcomes were similar to the cluster RCT study; however, the magnitude of the outcomes was delayed.ConclusionsThe implementation of pharmacy services is a complex multifactorial process, conditioned by numerous implementation factors. In the absence of remuneration, the implementation of the MRF service is a slow process, taking at least 12 months to complete.Trial registration numberCGFTRA-2017-01.

Stakeholder Experiences Implementing a National ROPS Rebate Program: A Grounded Theory Situational Analysis

Currently, little is known about what works, or does not work, in occupational safety implementation efforts. The aim of this study is to (a) explore what works and what does not in scaling up an agricultural safety intervention, and (b) explore these findings in terms of the Consolidated Framework for Implementation Research. A total of 13 stakeholders were interviewed about their experiences with the National Rollover Protective Structures (ROPS) Rebate Program implementation. Nine of these individuals also participated in follow-up interviews one year later. A Grounded Theory Situational Analysis approach was used for data collection and analysis. Two themes emerged from the data. First, the implementation strategy has evolved inconsistently across stakeholders (a barrier to implementation). Second, stakeholder engagement in the implementation is a function of perceived feasibility and “small wins” (an opportunity for improving implementation efforts). Based on the results of this study, two areas of needed improvement to the National ROPS Rebate Program implementation strategy were identified: (a) appropriateness and feasibility of inner setting stakeholder engagement, and (b) receptivity of outer setting stakeholders and potential funders. These findings will be helpful for increasing the success of the implementation, and can also provide guidance to others working on large-scale implementation studies.
