Performance-based financing (PBF) schemes have been expanding rapidly across low and middle income countries in the past decade, with considerable external financing from multilateral, bilateral and global health initiatives. Many of these countries have been fragile and conflict-affected (FCAS), but while the influence of context is acknowledged to be important to the operation of PBF, there has been little examination of how it affects adoption and implementation of PBF. This article lays out initial hypotheses about how FCAS contexts may influence the adoption, adaption, implementation and health system effects of PBF. These are then interrogated through a review of available grey and published literature (140 documents in total, covering 23 PBF schemes). We find that PBF has been more common in FCAS contexts, which were also more commonly early adopters. Very little explanation of the rationale for its adoption, in particular in relation with the contextual features, is given in programme documents. However, there are a number of factors which could explain this, including the greater role of external actors and donors, a greater openness to institutional reform, and lower levels of trust within the public system and between government and donors, all of which favour more contractual approaches. These suggest that rather than emerging despite fragility, conditions of fragility may favour the rapid emergence of PBF. We also document few emerging adaptations of PBF to humanitarian settings and limited evidence of health system effects which may be contextually driven, but these require more in-depth analysis. Another area meriting more study is the political economy of PBF and its diffusion across contexts.

National policy on global health (NPGH) arenas are multisectoral governing arrangements for cooperation between health, development, and foreign affairs sectors in government policy for global health governance. To explore the relationship between national and global processes for governing global health, this paper asks: in what forms of interaction between NPGH arenas and global health governance are learning and networking processes present? In a multiple case study of Norwegian and Swiss NPGH arenas, we collected data on intersectoral policy processes from semi-structured interviews with 33 informants in 2014-2015. Adapting Real-Dato's framework, we analyzed each case separately, producing monographs for comparing NPGH arenas. Analyzing both NPGH arenas for relational structures linking external resources to internal policy arena processes, we found five zones of interactions -including institutions, transgovernmental clubs, and connective forms. These interactions circulate ideas and soften arenas' boundaries. We argue that NPGH is characteristic of transnational governance of global health.

RESUMO O estudo buscou objetivar, por meio de uma revisão integrativa, em âmbito nacional e internacional, realizada entre 2012 e 2017, o conjunto de abordagens teórico-metodológicas para análise política em saúde, analisando tendências da produção de pesquisas no que se refere à construção teórico-metodológica. A síntese identificou um conjunto heterogêneo de autores e enfoques teóricos; objetos de estudo; estratégias de investigação e aspectos concernentes ao exercício da reflexividade na produção do conhecimento. A maior parte dos trabalhos é oriunda da saúde coletiva, apoiada nas teorias de matrizes sociológicas e das ciências políticas, como estudos sobre a gênese de políticas de perspectiva bourdiesiana e estudos embasados no institucionalismo histórico. Os estudos empíricos utilizam como estratégia de pesquisa a combinação de análise documental com entrevista de informantes-chave, em sua maioria. Poucos autores realizam um retorno reflexivo sobre sua postura teórica frente às demais posições no interior do espaço de produção da análise política em saúde. O estudo poderá subsidiar um debate que permita analisar a adequação, a pertinência e o alcance das abordagens como forma de fortalecer a análise política em saúde, considerada recente no Brasil, cujo potencial de diálogo teórico precisa ser estabelecido entre as distintas tradições.
