Objectives Previous research has shown that the self-consciousness and sexual function domain of the uterine fibroid symptom and quality of life (UFS-QoL) questionnaire exhibited poor adaptability and clinical applicability. In light of this, our objective was to develop a shorter version of the UFS-QoL questionnaire, while ensuring its reliability, validity, and clinical applicability, in order to enhance routine clinical assessments. Methods A secondary analysis was conducted on a prospective cohort study involving 20 Chinese hospitals and 2,411 Chinese women with fibroids. We employed both classical test theory (CTT) and item response theory (IRT) to select two items for each UFS-QoL subscale. In cases where inconsistencies arose from these two methods, we utilized responsiveness to identify items with a higher ability to detect change in clinical therapy. The short-form scale was evaluated for dimensionality, internal consistency reliability, and criterion and known-group validity through factor analysis, Cronbach's alpha, Spearman correlations, and independent samples t-tests. To assess the ability to detect change, we compared pre-treatment and post-treatment (6 and 12 months) scores using effect size and standardized response mean. Results Considering the poor adaptability and clinical applicability, we made the decision to remove the self-consciousness subscale and retain only one item from the sexual functioning subscale. As a result, we administered and validated the 11-item short-form version of UFS-QoL (UFS-QoL-11). UFS-QoL-11 exhibited a strong correlation with UFS-QoL in each subscale, with correlation coefficients exceeding 0.7 (all P < 0.001). Exploratory factor analysis identified three subscales (symptom severity, concern, and daily life interference) with eigenvalues > 1 for UFS-QoL-11. The test items accounted for 64.46% of the total variance, slightly higher than the parent scale score of 63.60%. UFS-QoL-11 demonstrated excellent internal consistency and reliability across all subscales (> 0.7), representing a slight improvement compared to the full UFS-QoL. Furthermore, UFS-QoL-11 exhibited sensitivity in detecting varying levels of current health status, particularly at 6 and 12 months post-surgery. The responsiveness of UFS-QoL-11 was comparable to that of UFS-QoL, with all effect sizes > 0.5, demonstrating a strong ability to detect changes. Conclusion UFS-QoL-11 performs as well as the original version, while reducing the administrative burden by 70%. We firmly believe that the implementation of the newly developed UFS-QoL-11 has the potential to facilitate widespread, accurate, timely, and sensitive monitoring in both clinical and research settings.

This review examined the effectiveness of ulipristal acetate (UPA) compared to leuprolide acetate (LEU) in the treatment of symptomatic uterine fibroids. The primary objective was to analyze relevant studies that investigated the improvement of symptoms, complications, alternative treatment options, and surgical outcomes for fibroids. Using the PICO format as a research approach, various aspects were compared for each treatment, including diagnostic accuracy, side effects, quality of life, and patient satisfaction measured through the Uterine fibroid symptoms and quality of life scale. UPA proved to be a safe, well-tolerated, and effective treatment option for symptomatic uterine fibroids, leading to amenorrhea and reduced heavy menstrual bleeding. On the other hand, LEU also effectively reduced fibroid size but had a higher incidence of adverse events such as hot flashes, vaginal dryness, and decreased bone density. It is crucial to consider all the aforementioned factors when selecting the most suitable treatment option for individual patients, as each treatment has its own advantages and disadvantages, including dosing requirements, side effects, endometrial changes, and their impact on treatment adherence and outcomes. Currently, there is limited evidence supporting the superiority of UPA over other drugs like LEU in short-term therapy for symptomatic uterine fibroids. Further research is necessary to establish UPA as a reliable and promising alternative. Treatment decisions should be individualized, taking into account patient comorbidities and considering both surgical and conservative approaches.

Comparative Efficacy of Uterine Artery Embolization versus Laparoscopic Myomectomy in Treating Uterine Fibroids: A Propensity Score Matched Analysis

Objective: The aim of this study was to comparatively analyze the therapeutic effects of uterine artery embolization (UAE) and laparoscopic myomectomy (LM) on uterine fibroids to determine which treatment method is more beneficial for patients. Materials and Methods:A retrospective study was conducted on 396 patients who underwent UAE(n=153)or LM (n=243)treatment from April 2010 to September 2019. After 1:1 propensity score matching(PSM), a comparative analysis was conducted on surgical trauma magnitude, postoperative recovery time, improvement in associated symptoms and quality of life, surgical adverse events, recurrence rates, and further interventions. Results: In PSM, 66 pairs (132 patients) were successfully matched. Both treatments significantly alleviated symptoms and enhanced quality of life. Compared to the LM group, the UAE group had less intraoperative bleeding (P<0.001), a lower rate of hemoglobin decrease (P<0.001), shorter operation, postoperative, and overall hospital stays (P<0.001), and a lower postoperative recurrence rate (P<0.05), all statistically significant. Moreover, the UAE group showed notable advantages in postoperative activities (P<0.05). However, UAE patients faced higher hospitalization costs (P<0.001). Adverse event rates (7.6% vs. 9.1%) and postoperative reintervention rates (7.6% vs. 7.6%) were relatively low and not significantly different between groups (P>0.05). Conclusion: Both UAE and LM can significantly improve patient symptoms and enhance their quality of life, and both treatment methods have low rates of adverse events and reinterventions. Compared to LM, UAE treatment for uterine fibroids presents advantages such as lesstrauma, faster recovery, and lower recurrence rate,but has higher treatment costs.
