Pratishyaya is one among Nasagata Roga (Disorders of the nasal cavity). Symptoms of Pratishyaya resemble those of Rhinitis. Childrenare more likely to develop Pratishyaya, which disturbs their daily activities and may hamper their growth and development. There is no use ofantibiotics in acute rhinitis. There is limited data regarding the safety of Pseudoephedrine and phenylephrine in rhinitis. There is a need to findthe best Ayurvedic interventions to prevent and cure Pratishyaya (Rhinitis). Previous review articles were alike and included the conceptual studyof Pratishyaya. Articles should have included proper classification, indications, and contraindications of Shodhana (Biocleansing therapy) andShamana (Palliative procedure). No review article focused on Pratishyaya and its management in the pediatric population. There is a need toclassify Shodhana karma and Samshamana therapy in Pratishyaya along with their indications and contraindications and to review the Pratishyaya inpediatric population. Disturbance of digestive fire and production of Ama (toxins) causes vitiation of Doshas and obstruction of channels whichproduces symptoms. Shamana treatment should be administered in the acute stage to digest vitiated Doshas and Shodhana therapies like Nasyaare to be given in the chronic or recurrent stage of Rhinitis. Improper milk intake and sleep intake can lead to Pratishyaya in breastfeeding babies.Dhoopana (herbal fumigation of the nasal cavity) is indicated in the acute stage of Rhinitis. Langhana (Fasting or light diet) helps correct Agni andvitiated Doshas. In Dhoompana, fumes are a gaseous medicine that increases its bioavailability. Nasya is effective but is not indicated for childrenbelow 7 years of age. So, local nasal administration of fumes (Dhoopana) can be adopted for its management.

Protracted chronic and acute rhinitis: approach to therapy

Introduction. The prevalence of acute and chronic rhinitis determines the socio-economic side of the issue and requires effective treatment and improvement of patients’ quality of life.Aim. To study the effectiveness of the Rinofluimucil® in patients with acute and chronic catarrhal rhinitis at the outpatient stage of treatment.Materials and methods. The study was conducted in a group of 82 patients aged 18 to 59 years (average age 37,7 ± 13,1 years) diagnosed with acute rhinitis and chronic catarrhal rhinitis. Two groups were identified: the main group (21 patients with acute rhinitis and 20 patients with chronic rhinitis) and the control group (20 patients with acute rhinitis and 21 patients with chronic rhinitis). The control group received elimination and symptomatic therapy, the main group received endonasal monotherapy with Rinofluimucil®, one dose 3 times a day for 7 days. The effectiveness of the treatment was determined by the dynamics of subjective and objective symptoms.Results and discussion. In patients with acute rhinitis in the main group, the assessment of the amount of nasal discharge and concentration of attention changed by 12,3 and 10,5 times, respectively. In patients with chronic rhinitis, the assessment of the amount of nasal discharge and concentration in the main group changed by 3,3 and 2,6 times, respectively. Restoration of mucociliary clearance to normal with the use of the Rinofluimucil® was observed in patients with both acute rhinitis (18,1 ± 0,4 s) and chronic catarrhal rhinitis (21,3 ± 0,2 s). According to the PARM data, by the 7th day of observation there was an improvement in the average value of the total volumetric flow of nasal breathing in patients with acute and chronic rhinitis in the main group by 1,9 and 2 times, respectively.Conclusions. The data obtained indicate the importance of using the Rinofluimucil® in patients with acute and chronic rhinitis in clinical practice.

Treatment Regimen for Acute Viral Rhinitis in Patients with a History of Rhinitis Medicamentosa

Aims: The aim of this research is to study the efficacy of intranasal administration of 0.9% saline solution in patients with acute viral rhinitis and a history of Rhinitis medicamentosa. Materials and Methods: The study included 96 patients with a confirmed diagnosis of acute viral rhinitis and a history of Rhinitis medicamentosa. All patients were equally randomized to two groups. In both groups, patients were treated according to the European Position Paper on Rhinosinusitis and Nasal Polyps with nasal decongestants (oxymetazoline), but in Group 2, the treatment regimen was supplemented with topical use of 0.9% saline solution. Local TNF-α and IL-1β levels were determined in all patients on Days 1, 5, and 10. In addition, on the next day after oxymetazoline withdrawal, a Nasal airway resistance was measured in all patients using active posterior rhinomanometry. Results: The duration of rhinorrhea and nasal congestion and, respectively, the duration of oxymetazoline administration significantly differed between the groups (p<0.001) and was 7.9±1.1 days for Group 1 and 4.7±0.9 days for Group 2. In general, the dynamics of changes in local TNF-α and IL-1β levels in both groups was similar. Throughout the study, there was a progressive decrease in both inflammatory mediators, with faster changes occurring in Group 2. A strong positive correlation (rs=0.89; p<0.001) between TNF-α and IL-1β levels was established. According to the results of active posterior rhinomanometry on the day following oxymetazoline withdrawal, Nasal airway resistance was significantly higher in Group 1 (p<0.001), which indicates the presence of difficulty breathing in patients of this group. In addition, patient examination showed that manifestations of Rhinitis medicamentosa occurred in 3 (6.25%) patients of Group 1 and in 17 (35.42%) patients of Group 2. Differences between the groups were significant (p<0.01). Conclusion: The use of topical nasal decongestants, in particular oxymetazoline, in patients with acute viral rhinitis and a history of Rhinitis medicamentosa for more than 7 days has a potential risk of development of a recurrence of Rhinitis medicamentosa. Supplementing the treatment regimen for acute viral rhinitis with the topical use of 0.9% saline solution reduces the duration of treatment and the use of topical decongestants, and therefore prevents the recurrence of Rhinitis medicamentosa.
