Acute Type A Aortic Dissection Involving the Left Main Trunk of the Coronary Artery

Abstract: This report describes 2 cases of a type A acute aortic dissection combined with myocardial infarction caused by a retrograde dissection into the left main trunk of the coronary artery. Successful surgical treatments, including the replacement of the ascending aorta, aortic valve resuspension and coronary artery bypass grafting, were performed in both patients, and they recovered well from cardiogenic shock. However, left ventricular function of both patients remained depressed postoperatively, which limited th… Show more

“…Although aortic dissection involving the left coronary artery occurs less frequently than aortic dissection involving the right coronary artery, it is more frequently lethal and more urgent [4][5][6][7]9) .…”

“…Although a definite method for treating an infarcted myocardium due to acute aortic dissection has not yet been established, it is extremely important to restore coronary circulation immediately [4][5][6][7][8] . The effectiveness of the preoperative placement of a perfusion catheter or a coronary artery stent into the ischemic coronary artery has been demonstrated.…”

“…Several case reports of a Stanford type A aortic dissection in combination with a myocardial infarction have been published. [1][2][3][4][5][6][7] In almost all cases the right coronary artery was involved. [1][2][3] We report a case of anterior myocardial infarction due to a Stanford type A aortic dissection involving the left main trunk of the coronary artery.…”
