Acute Spinal Intradural Extramedullary Hematoma: A Nonsurgical Approach for Spinal Cord Decompression

Abstract: The authors present the case of a 60-year old man with a spontaneous spinal intradural hematoma in the thoracic and lumbar region, which was caused by anticoagulant therapy and led to a severe progressive transverse lesion. After substitution of coagulation factors, a small catheter was inserted into the subarachnoid space via a lumbar puncture. By alternating irrigation and suction removal of the blood clot, restoration of the cerebrospinal fluid passage was possible along with a marked improvement in the neu… Show more

“…Conservative treatment is possible if an early recovery is observed,7 , 10 , 13 , 16 , 25 , 3739 if the deficits are mild, if the degree of extension precludes surgical treatment,39 or if a coagulopathy is associated 13 , 15. In our case, the presence of blood in both spaces was not confirmed surgically.…”

“…6,8,9,11,12 In all nonsurgically managed cases, the initial severity of neurologic symptoms was grade D or E on the Frankel grading scale, or improvement was noted within 2 or 3 days. Based on our experience and on published reports, we conclude that surgery is not essential in all cases of SSH.…”
