Acute Safety and Efficacy of a Novel Multipolar Irrigated Radiofrequency Ablation Catheter for Pulmonary Vein Isolation

Abstract: PVI using the novel irrigated RF multipolar ablation device (nMARQ™) appears to be acutely effective. No clinical complications were identified. A high incidence of SCL (33%) and thermal esophageal lesions (33%) bears caution and further studies on long-term efficacy and safety are needed.

“…Several one-shot catheters have been developed, including balloon-based technology and multielectrode ablation catheters [4][5][6][7]. At present, there is no data analyzing the influence of PV anatomy on the clinical efficacy of the irrigated multielectrodes ablation catheter (nMARQ TM ) for PV isolation in patients with AF.…”

“…Touch-up ablation using a 3.5-mm tip open-irrigated catheter was needed only for 1 patient (2 veins) as reported by Scaglione et al [7] but not in any of the case reported by Shin et al [5]. One left superior PV and 1 left inferior PV could not be isolated using the nMARQ TM catheter as reported by Deneke et al [8]. However, is not mentioned if touch up ablation was performed.…”

“…In all cases except one left inferior PV, confirmation of PVI was achieved using only the nMARQ TM catheter as reported by deneke et al [8]. However, the concordance between the nMARQ TM signals…”

“…Preliminary reports raised concerns about the safety of the nMARQ TM catheter. Deneke et al (7,13) reported one esophago-pericardial fistula, a high incidence of mild thermal esophageal lesions, and an incidence of silent cerebral lesions higher than single-tip irrigated RF. However Scaglione et al (12), using a specific and cautious procedural workflow, observed the complete absence of periprocedural silent cerebral ischemia.…”
