“…A range of 59-70% of HIV-PAH cases are reported to be in individuals engaging in IVDU (14)(15)(16). Although the abuse of cocaine and other stimulants may be an independent risk factor in the development of pulmonary vascular injury (17)(18)(19)(20), HIV-1 and cocaine interplay in the host might be involved in the exacerbation of pulmonary vasculature dysfunction, leading to the higher incidence of PAH in HIVinfected individuals with IVDU as a risk factor. Clearly, HIV and cocaine each have the capacity to orchestrate endothelial cell injury and dysfunction that may lead to abnormal smooth muscle cell (SMC) proliferation/migration and subsequent vascular remodeling.…”
