Acute rectal obstruction after PPH stapled haemorrhoidectomy

Abstract: An unusual case of acute intestinal obstruction following a PPH stapled haemorrhoidectomy is reported: the complication was due to an almost complete interruption of the rectal lumen tightened by a purse-string suture anchored by staples. The stricture was dilated and the suture released thus restoring intestinal continuity. Surgeons should be aware that such troublesome complication may occur and can be treated successfully by a transanal approach.

“…14 This was also encountered by us and was clinically manifest on the 5th postoperative day with features of generalized peritonitis. Other complications such as pneumoretroperitoneum, 15 pelvic sepsis 16 and rectal obstruction 17 have also been reported but are fortunately rare and were not encountered by us. SH is perhaps the most exciting new development in the definitive treatment for haemorrhoids.…”

“…19 There are 2 case reports of rectal perforation after stapling procedures for hemorrhoids. 22 Acute intestinal obstruction because of the closure of the rectum by the purse string suture is also known. 23 In our study, there were no such complications supporting the view that complications probably occurred as a result of lack of adequate experience and can be overcome once the learning curve is passed, a short one at that for stapler hemorrhoidectomy.…”

“…(18) This was also encountered by s.s.jaiswal et al and was clinically manifest on the 5th post-operative day with features of generalized peritonitis in their study. Other complications such as pneumoretroperitoneum , (19) pelvic sepsis (20) and rectal obstruction (21) also been reported but they are fortunately rare and were not encountered by us. HETZER et al (13) found no difference in the occurrence of recurrent haemorrhoidal disease when comparing the SHand conventional procedures.…”
