Acute promyelocytic leukemia and pregnancy. A case report

Abstract: Background. Acute promyelocytic leukemia (APL) is an uncommon form of acute myeloid leukemia usually associated with disseminated intravascular coagulation (DIC). Pregnancy in patients with APL requires special consideration to maximize the probability of survival of both mother and fetus. Methods. A patient with APL diagnosed during pregnancy who developed DIC is described. Obstetric and oncologic management of this difficult patient is discussed, and a pertinent literature review of pregnancy in APL is prese… Show more

“…Patients should be closely monitored for clinical and laboratory manifestations of this coagulopathy. Prior to the ATRA era, APL treated with standard chemotherapy did not always result in live birth . However, the current management of APL during pregnancy remains potentially complicated by the potential teratogenic effect of chemotherapy, ATRA, and arsenic trioxide (ATO).…”

“…APL is recognized to occur infrequently during pregnancy. APL is characterized by an onset at young age and life-threatening bleeding diathesis attributed to disseminated intravascular coagulation (DIC)-like coagulopathy [61,62]. The discovery of all trans-retinoic acid (ATRA) has changed the course of APL treatment by reducing the onset of DIC and inducing complete and durable remissions in more than 90% of patients [61].…”

“…Acute leukemia during pregnancy is very rare with an estimated frequency of 1 in 75,000-100,000 [3, 6, 7]. AML accounts for approximately two-thirds of leukemia cases seen during pregnancy and the diagnosis is generally made during the second and third trimesters [3, 5].…”

“…The exact incidence of acute promyelocytic leukemia (APL) in pregnant women is unknown; however, approximately 50 such cases have been reported in the literature [1-4, 7, 8]. APL is a unique type of acute myeloid leukemia (AML) characterized by proliferation of neoplastic promyelocytes bearing a specific chromosomal translocation t(15;17).…”
