Acute pancreatitis complicated by acute myocardial infarction – A rare association

Abstract: Acute pancreatitis complicated by acute myocardial infarction has been reported very rarely. The exact mechanism of the cause of myocardial injury is not known. We report a case of 36 year old male presenting with acute pancreatitis complicated by ST elevation acute myocardial infarction (AMI). The administration of thrombolytic therapy in such patients can have deleterious effects. We report successful performance of primary angioplasty in this complicated patient.

“…Acute coronary syndrome was a rare event in our cohort (1.8%, 3/163), which is in keeping with several published case reports [ 28–31 ]. The incidence is lower than in similarly morbid conditions such as sepsis, where the incidence has been reported as 4.1% [ 32 , 33 ].…”

“…The following hypotheses have been proposed to explain these ST-segment abnormalities in ECG: (1) Hypovolemia and hypotension can cause coronary hypoperfusion especially in patients with significant coronary artery disease [ 10 ]. (2) Electrolyte abnormalities such as hypokalemia, hypomagnesemia, hypocalcemia, and hyponatremia can modify the repolarization phase on ECG [ 6 ].…”

“…Acute pancreatitis associated with true myocardial infarction is a rare incident and Kumara et al [ 8 ] have reported such a true ST elevation myocardial infarction in a young patient with acute pancreatitis. In 2005, Krantzopoulos et al [ 9 ] also have reported another case of myocardial infarction with acute pancreatitis.…”
