Acute normovolemic hemodilution reduced allogeneic blood transfusion without increasing perioperative complications in patients undergoing free-flap reconstruction of the head and neck

Abstract: Purpose The present case-control study was conducted to evaluate whether acute normovolemic hemodilution (ANH) can reduce the need for perioperative allogeneic blood transfusion (ABT) and affect the incidence of perioperative complications in free-flap reconstruction of the head and neck. Methods This single-center, retrospective, observational study included the perioperative data of 123 patients who underwent free-flap reconstruction of the head and neck following oncological surgery. Patients were divided i… Show more

“…There are two main types of autotransfusion: the ANH, which is conducted during operation without preoperative blood collection ( 18 ), and the predonated transfusion, which is prepared and collected preoperatively ( 19 ). In our study, we compared the two types of transfusion and found that predonation is associated with shorter hospital stay, lower blood loss, and smaller decrement of Hb change.…”

“…showed that ANH might recover up to 70% of the shed blood in orthopedic surgery and significantly reduce the use of erythrocyte [ 9 ]. Subsequently, lots of literatures demonstrated that ANH is an effective alternative strategy [ 10 , 11 ].…”

“…Acute normovolemic hemodilution (ANH), another autologous blood transfusion strategy, has been reported to reduce exposure to perioperative ABT in patients undergoing gynecological cancer surgery (9). Meanwhile, ANH use doesn't increase incidence of post-operative complications in patients undergoing free-flap reconstruction of the head and neck (10). Compared with preoperative autologous donation (PAD) of 400 mL one week before surgery, ANH protocol has similar postoperative hematologic outcomes (11).…”
