Acute Labyrinthitis Revealing COVID-19

Abstract: An 84-year-old man presented to the emergency department for acute vomiting associated with rotational vertigo and a sudden right sensorineural hearing loss. A left peripheral vestibular nystagmus was highlighted. The patient was afebrile, without respiratory signs or symptoms. Blood sampling at admission showed lymphopenia, thrombopenia and neutrophil polynucleosis, without elevation of C reactive protein. “…A cerebral MRI was performed in 10 of the 20 patients with audio-vestibular disorders. The neuroimaging revealed a clear inflammatory process of the inner ear in two of them, in one case bilateral, supporting the hypothesis of a direct cochlear and labyrinth damage (Degen et al 8 and Perret et al, 19 ).…”

“…In addition to hearing loss, all reported cases noted additional symptoms, including tinnitus (36%), ear discomfort (15%), headache (14%), and nausea (12%) ( 5 ). Currently there are only few published case reports regarding the manifestation of otologic symptoms after COVID-19 infection, and no published correlations between hearing loss or other otologic symptoms after immunization with any available COVID-19 vaccines ( 6 ).…”

“…Perret et al described a case of acute labyrinthitis and SSNHL ( 29 ); the patient was treated with oral prednisone and showed progressive recovery. The oral steroid treatment was effective also for treating the hearing impairment in a patient on peritoneal dialysis program who showed SSNHL as presenting symptom of SARS-CoV-2 infection ( 29 ). An improvement of the hearing capacity was also reported by Abdel Rhman and Abdel Wahid ( 30 ); in this case the patient was treated with intratympanic steroids.…”
