Acute Kidney Injury, Risk Factors, and Prognosis in Hospitalized HIV-Infected Adults in South Africa, Compared by Tenofovir Exposure

Abstract: There are limited data describing acute kidney injury (AKI) in HIV-infected adult patients in resource-limited settings where tenofovir disoproxil fumarate (TDF), which is potentially nephrotoxic, is increasingly prescribed. We describe risk factors for and prognosis of AKI in HIV-infected individuals, stratified by those receiving and those naive to TDF. A prospective case cohort study of hospitalized HIV-infected adults with AKI stratified by TDF exposure. Adults (≥18 years) were recruited: clinical and bioc… Show more

“…A recent study in South Africa showed an incidence of renal impairment of a little below 3% during the first 12 months of TDF-containing ART in primary care populations, 29 while another South African study of patients hospitalised with AKI reported TDF therapy to be associated with more rapid worsening of renal function, a higher likelihood of proteinuria and acidosis, and delayed renal recovery. 30 The latter study highlights the poor prognosis of AKI in HIV-infected individuals in the context of limited access to renal replacement therapy, as more than a quarter of the patients in this cohort died. 30 …”

“…Table 3 summarises the findings and conclusions made by the various authors from their studies. About 90% of the studies focused on chronic outcomes of kidney function in patients on TDF-containing ART while two reported on acute outcomes [34, 35, 51]. Most of the studies reported at least some incidence of RD in the patients taking TDF containing ART.…”

“…There is no study in Africa as well as in Ethiopia to show the extent of fall in eGFR greater than 25% relative to baseline. However, a prospective case cohort study in South Africa showed that among admitted patients with AKI 61% were on TDF based antiretroviral regimen [21].…”
