Acute Inferior Myocardial Infarction Caused by Lightning Strike

Abstract: Lightning strike is an infrequent natural phenomenon with serious medical complications, like multiple organ damage, and it is associated with increased risk of mortality. Cardiovascular complications are among the most hazardous complications of lightning strike. Lightning strike can cause various serious consequences ranging from electrocardiographic changes to death. We reported a 21-year-old patient with no cardiovascular risk factors struck by lightning and presented by inferior ST elevated myocardial inf… Show more

“…Inferior wall was the most common side affected among the surveyed population ( 88 from occlusion of the coronary artery that results in decreased perfusion to the inferior myocardium. MI involving the inferior wall occurs in approximately 40% of all MIs and it is associated with a better prognosis than MIs in other regions, such as anterior MI (27)(28)(29). However, these findings were self-reported and may lack accuracy, which is a limitation of the study.…”

“…A person struck by lightning may have injuries that are apparent, or they may be struck and killed without visible injury. The most common causes of death by lightning are dysrhythmias, such as ventricular tachycardia or ventricular fibrillation, 2,8,9 myocardial infarction, 10 12 and respiratory arrest caused by central nervous system effects. 2 Barotrauma may cause pneumomediastinum, rupture of the tympanic membrane, and injuries to the gastrointestinal tract.…”

“…As the lightning discharge passes through the body, it can damage various organs and systems. 6 Case fatality rates due to lightning vary between 10 and 30%, and survivors may develop serious complications such as peripheral neuropathy and defects in cognitive functions. 7…”

“…Sometimes, there are symptoms of burns on clothes and body and fernlike markings specific to lightning strikes on their bodies, acute kidney failure, cardiac arrhythmias and deaths. 1,6,8 The internal organs may show oedema, congestion, etc, but such findings on their own are not diagnostic of death from a lightning strike. It is essential for accurate diagnosis to examine the person’s clothes, external body and conduct a crime scene investigation as well.…”
